The study of consumer behavior as a separate marketing discipline all started when marketers realized that consumers did not always react as marketing theory suggested they would (Ekström, 2003). Many consumers rebel at using the identical products that everyone else used, instead they prefer differentiated products that they feel reflect their own special needs, personality and lifestyles.

The reason for the controversy of the Hospital Consumer Assessment of Healthcare Providers & Systems, referred to as HCAHPS (pronounced “H-caps”), is the tie that the Center for Medicare and Medicaid Services (CMS) placed between the scores of the assessment and healthcare reimbursement (Westbrook, Babakus, & Grant, 2014). There are two sides to consider when addressing HCAHPS/Press Ganey surveys as they directly affect hospital reimbursement. Patient satisfaction, quality of care, and how they portray their hospital stay contributes to the reimbursement that hospital receives. The nurse-patient relationship plays a large role in influencing the quality of care than patients feel that they are receiving.
Health care is an uprising issue today in the United States. I believe in order for health care or the medical field to succeed in the future that social contract should be enforced. By enforcing social contract, it will allow health care to be more efficient by allowing individuals to assume responsibility for their own healthy by having the ability to ensure health. According to The Enduring Democracy book, " from the philosophy of Jean- Jacques Rousseau, an agreement people make with one another to form a government and abide by its rules and laws, an in return the government promises to protect the people’s rights and welfare and promote their best interest"(Dautrich, 7). In other words, if people came to an agreement about health care being available for all American citizens, the government will uphold this idea and will make sure all American citizens have the right to health care.
The aim of this assignment is to view the Panorama documentary (BBC,2014) with regards to the quality of care given. The documentary provides an observation of health and social care environments; it shows how service users are mistreated on a daily basis. It also aims to critically analyse changed made to healthcare policies. It how these changes are implemented in practice to improve the service provided and protect clients.

There is no doubt that while the health care system attempts to meet many of the needs of the community, it cannot plan and implement a health care system which caters for the diverse needs of the wider community without consumer participation. Community participation is crucial in determining equity in a service which is more accessible to those of a higher SES and a higher education. Acknowledging the specialised health care needs of some individuals only becomes possible after consumer participation. Becoming involved and bringing a different perspective can only be a positive contribution to health care.
It involves health care transformation where by patient passively participating in their health care process to the best of their interest in order to live a quality and healthy life to their expectation. I value the healthcare process as a team work between patients and health care providers. The patient participation concept applies and provides me with a solid foundation towards my beliefs and values in my further practice as cardiothoracic surgery nurse practitioner. I will utilize the attributes to establish a good, trusting, respectful relationship with surgical patients by surrendering some power to patients by negotiating, sharing responsibility, and viewing patient equally. I will also share information and knowledge, provide education based on patients’ experiences, respect patients’ opinions, and expectations. Patient and the health care team will actively engage in intellectual and physical activities. Through this analysis, I am confident that the concept of patient participation will guide my future practice, it aligns with my health care beliefs and values. It provides benefit in the relationships between health care providers and the patient population. Participation is stereotypically seen as something just for patients, however using the public is just as important. There is often an idea that patients can only get involved in clinical trials and that any opinions they may have about their research should be held back as ‘the doctor knows best’. This phrase can often be heard throughout members of the public.
