Patient participation has been poorly defined especially in nursing practice due to lack of freedom from indistinctness or ambiguity in health care (Sahlsten, Larsson, Sjostrom, & Plos, 2008). The concept of patient participation plays an important role in health care nationally and globally. The meaning of patient participation varies greatly based on individual patient and nurse’s perception. This concept analysis it to gain clarity through Walker and Avant concept analysis method by identifying and presenting the purpose, attributes, model case, antecedents, consequences and empirical referents of the concepts.

It involves health care transformation where by patient passively participating in their health care process to the best of their interest in order to live a quality and healthy life to their expectation. I value the healthcare process as a team work between patients and health care providers. The patient participation concept applies and provides me with a solid foundation towards my beliefs and values in my further practice as cardiothoracic surgery nurse practitioner. I will utilize the attributes to establish a good, trusting, respectful relationship with surgical patients by surrendering some power to patients by negotiating, sharing responsibility, and viewing patient equally. I will also share information and knowledge, provide education based on patients’ experiences, respect patients’ opinions, and expectations. Patient and the health care team will actively engage in intellectual and physical activities. Through this analysis, I am confident that the concept of patient participation will guide my future practice, it aligns with my health care beliefs and values. It provides benefit in the relationships between health care providers and the patient population. Concept analysis is essential in nursing to produce a more in-depth understanding of the concept being analyzed; thus, improving nursing practice. Over the past few years, the health care service has seen many changes. The Affordable Care Act, for example, creating more insurance in order to care for the indigent and people in the most need of help. Health care is a very essential and necessary element of an individuals lives. The methods and preparation that is needed in order to provide adequate and efficient patient care to all is very critical and sometimes specific. The health care organization has ventured from focusing on input management to focusing and improving output management (White, 2011).
A team led by RN, should mentally prepare their patients to understand their responsibility towards good health. This can be done by showing them special documentaries during their stay in the hospital, in a common room where other patients can also join them in a group of six to twelve. After the session, patients should be given a short comments form with multiple choice answers (Appendix A). The purpose is to check their positivity towards the message conveyed through the documentary. At this time patient's vitals should be checked and recorded for the future
Good teamwork is important in a patient centred care. It is a team of health professionals who actively participate, cooperate, interact, communicate expertise, respect, trust and its main focus is to improve patient’s health (Miller, 2008, p.14). Also, the team includes the family of the client and the patient itself (Miller, 2008, p. 15). Therefore, all members have a role to play. For instance, in the nursing practice it involves health promotion and maintenance regarding patient’s health in order to decrease the impacts of negative outcomes (NMBA, 2010). Nevertheless, this can be maintained under the national competency standard (NMBA, 2010). Part of the national competency standard promotes professional responsibility, multidisciplinary approach, critical thinking and client care delivery (NMBA, 2010).
The Patient Self-Determination Act (PSDA) was implemented to allow patients to state “Do Not Resuscitate” (DNS), or to assign a surrogate decision maker in the event the individual is unable to make the decision. Closed cardiac massage was introduced in 1960 and still the health care provider operated in fear on who to actively resuscitate and they would sluggishly respond on patients who may not benefit based on extent of illness (Ball, 2009). The care providers operated in fear of litigation depending on the family’s response but was fairly relieved by introduction of the Patient Self-Determination Act (PSDA).
In an interview with a staff nurse (S.N), the main problem within patient communication included lack of patient’s (and family) involvement/willingness in planning cares. The staff nurse emphasized how “Patients often feel overwhelmed and do not want to participate. But, it is important for patients to be involved in their care for better outcomes” (S.N., personal communication, February 5, 2014). The staff nurse’s statement is supported by Evans (2013) whom remarked “better-informed patients avoid unnecessary care and frustration”.
The Patient self- Determination Act (PSDA) was legislated by Congress in 1990 as part of the Omnibus Budget Reconciliation Act (OBRA). Congress felt that individuals has the right to determine their final healthcare. The PSDA provides every competent adult and emancipated minor with the right to decide their own decision on what medical care or treatment they accept, reject, or discontinue. The effect of the PSDA has given each patient the right to choose what is best for them upon their beliefs.
