Chronic illness and patients suffering from injuries with long recoveries are a complex patient population. In 2012 it was estimated that 117 million had one or more chronic health conditions (Center for Disease Control and Prevention, 2015). Patients with chronic illness and injury require early intervention, coordination of multiple disciplines and they have healthcare needs which are not easily addressed with current healthcare model. The purpose of this paper is to introduce the potential investor to Whole Body Center where we embrace new approach in which chronic issues are not just assessed by a single medical view—rather we provide care with a whole body approach as chronic conditions are multifaceted. I will present the evidence as to why Whole Body Center is needed by going over an in depth SWOT analysis detailing the strengths, weaknesses, opportunities, and threats. I will used this analysis to identify goals supporting the position of Whole Body Center. A market analysis has also been included which details the current client bases as well as identifying the needs and gaps in the current system, the business model as detailed by the business model canvas and the financial overview with a 3-year projection of revenues. Lastly I have identified areas of measurement and evaluation to continue to focus on providing the best care for our patient population including early identification of health initiatives by the Institute of Medicine (IOM), Healthy People 2020, and other government health organizations. Whole Body Center has identified transparency, inspire, holistic, health pioneer, and compassion as the foundational values of the program when maneuvering through the complex healthcare system and are based on Lowney's (2003) four pillars for successful leadership of self-awareness, ingenuity, love, and heroism. Transparent: Our patients will have a clear vision of health goals, plan of care and the patient and provider relationships. Providers will work actively to Inspire: Encourage and support each patient while identifying goals and personal potential.
