The “My Life” Service Integration Model was innovated and developed as a part of the Finnish national experimental development project leaded and funded by the Ministry of Finance and a subproject of the Municipality of Kuopio and its partners.
The Finnish welfare state and the public service system is based on multiple legislations, organizations, fundings, technology and customer data records. This has led to overlapping and parallel services and processes for the multiple service users and the professionals. In this project Kuopio innovated, tested and developed a new model called My life for the human service integration for young adults. As a result My life service integration model showed that the more young people feel that they are listened to, the more they can participate and the more they can give feedback on the services they use, the better is their feeling of empowerment. Young people also believe that the same model could also serve other young people
The aim of Kuopio’s new My life model was to include service integration and a case manager method as well as to bring the separate service plans together. Another aim was to construct an integration model as digital and customer inclusive as possible. During two years 17 professionals and 34 youngsters innovated, tested and evaluated the model together. The co-creation project was in 2015-2016 and was based on the experimental developing.

The assessment was co-created with docent S. Kainulainen from the Diaconia university of applied sciences Helsinki and the professionals and the young adults form the City of Kuopio. Docent Kainulainen is an expert of the young adult life and their challenges. The very first prototype of the assessment was based on the theories. After the pretests with the young adults, the assessment was reinforced. The final “3X10D Survey” consists of 10 themes as studies/work, family, health, friends, personal development, housing, finance, life satisfaction, self-esteem and resilience. The customer considers each theme from three different viewpoints: importance, satisfaction and future. Being aware of what life course one’s on and the impact of transitions within a person’s life course is important for social work practice. It is important that social workers reflect on both their own and other’s experiences throughout the life course and support this understanding within the various theoretical frameworks. It helps us to understand the common themes which have affected people through the age and helps us understand why people behave the way they do in certain situations.“Understanding of life course aims to encourage us in the flexibility of imagination which is required in order to enter into the lives of people at different ages and to see the world from their point of view” (Sudbery, 2010, p.231).

Introduction to human services has given me a glimpse of what the human services professions do, the task are not always easy. They are committed to helping clients become empowered, by helping them to grow to becoming whole, and functional.
Person centered approach: In this case, choices, needs and preferences of person are supported. Person’s requirement is the most important factor according to this approach. Approach of one person may vary with another person. Person centered approach is needs led rather than services led.
There remains this experience in my mind that I need to write about that actually inspired me in countless behaviors in reference to service and self. I consider relationships concerning those in the helping field and those obtaining aid stands as particularly vital; hence, I believe I must direct focus on this concept. I resolved to enter this discipline because I wanted to give back to those in the community in need. One day I was speaking with a counselor at Centaur who happened to tell me about the significance of connecting to clients, and she expressed to me that I should pay homage to those who accept the “gift” of service because without them, we would not possess the zeal to accomplish authentic behaviors approaching community service. I realized these were true words of wisdom because when I thought about it, I realized my actions were not just about those that I helped but how I helped myself. There were quite many relations made between my service learning experiences and themes addressed in the course. Some of the connections were about human self-respect, unity, and fairness. The social ties hold people together and can support the people who do not have the power to help each other. It was interesting for me to employ my skills and knowledge I learned in the classroom to serve those who need help.
Provision of integrated services was noted as innovative. A situation analysis is done before conducting the integrated services to find out who are they, where are they located and why are not being immunized or being missed. The next step is the integration immunization services with other programmes such as, Malaria (Insecticide Treated Nets) Vitamin A and plump nuts provision to increasing coverage , access and demand for immunization services with more focusing on rural remote areas , urban slums and hard to reach areas .
