Recent seekers of healthcare services such as governments, insurance companies and individuals face a contemporary global crisis with paying exorbitant costs for healthcare services. This prohibitive cost has made an impact on recent debates between supporters for the importance of the treatment and deniers because of the highly cost. In the light of these clashes between advocates and deniers, a gesture appears from behind the scene attempt to offer a phenomenon. This phenomenon claims that physical therapy could provide an alternative therapeutic protocol with a higher quality and less expensively. Every new phenomenon has its advocates and deniers. Evidence based practice should be the judge gavel. This paper will analyse the effectiveness of physical therapy approach by therapeutic exercise, electrical stimulation and relaxant modalities, and analyse five pharmaceutical approaches with comparing and contrasting both approaches for treating depression and anxiety. Therapeutic exercise has noteworthy benefits particularly in depression and anxiety. The strong point that no one can deny is the importance of exercise to human’s body in general. Therapeutic exercise will burn fats, enhance musculoskeletal, cardiopulmonary and nervous systems. Furthermore, a research carried out by Farrell et al. (1982 cited in Bender, et al. 2007: 373) found that a half hour of running exercise on treadmill raise b-Endorphin levels two to five doubles. This significant increase will stimulates the brain receptors. Which will reduce the feeling of pain accompanies by a positive outlook on life that will treat depression and anxiety. Ciraulo (2009: 33) classified these pharmaceutical approaches for treating depression and anxiety into terms frequently used in clinical situations. Ciraulo (2009: 33) listed these approaches as, “selective serotonin reuptake inhibitors (SSRIs), tricyclic antidepressants (TCAs), cyclic antidepressants, mixed action agents, selective norepinephrine reuptake inhibitors (SNRIs), monoamine oxidase inhibitors (MAOIs), and alternative (non-traditional) antidepressants.”

Treating depressive and bipolar disorders with antidepressants remains a popular option in clinical practice. Most clinicians choose the drug or class of drugs, usually selective serotonin reuptake inhibitors, SSRI's, that is most effective and best tolerated with fewer severe side effects. These drugs are beneficial because they specifically target serotonin-based areas of the brain without affecting other neurotransmitter systems. SSRI's largely replaced tricyclic antidepressants which work by blocking the absorption (reuptake) of the neurotransmitters serotonin and norepinephrine, thereby increasing the levels of these two neurotransmitters in the brain. Why can some people be in a perfectly good mood one day and then less than a week later start exhibiting the signs of clinical depression? I have always been curious about the role that experience and chemical imbalances play in depression and other mood disorders. Depression is a mental illness, which affects millions of Americans each year. Currently there are many prescription drugs, called anti-depressants that have been proven to successfully treat it. The causes of depression are somewhat of a medical enigma, however, it is known that depression is associated with a change in the brains chemistry involving the function of neurotransmitters (Reichert). This chemical change occurs in healthy brain’s, which experience sadness, but ends after the unpleasant stimulus is removed. In people suffering from depression this chemical change does not correspond to any particular stimulus. There is a fine line between anxiety and depression. A line that is often times blurred. Although there are differences between the two, they also share many similarities, which can lead to false diagnoses for patients. It only gets more complicated when both illnesses are present. For example, The National Institute of Mental Health (2009) did a study of anxiety disorders and found that 53.7% of people reported they also experienced major depression as a secondary condition. These researchers also stated that people who are severely depressed do become anxious. In order to have a better understanding of anxiety and depression one must first clearly define the two conditions, understand the causes, look at the symptoms involved, and review the different treatment options available.
