Depending on the mechanisms and forces involved, the anterior tibiofibular ligament can become sprained or even avulsed with a small fragment of bone from the tibia or fibula. Continued application of forces to the ankle, especially an external rotation force, can rupture the tibiofibular ligaments and interosseous membrane and possibly cause an oblique or spiral fracture to the fibula.
