Managed care is a system that focuses on providing the most cost effective plan for groups of clients through the partnership of health care providers and agencies.. This particular configuration is utilized in primary, team, functional and alternative nursing care delivery systems. A case management system uses a multidisciplinary team and collaborates on coordinating the most effective procedure, and a case manager may even be a nurse or social worker. An early developed model of nursing is case method and involves one nurse assessing a whole group over the course of a 8-12 hour shift. Functional method is a task oriented approach which can overlook the emotional needs of the patient. This model is based on production and efficiency. Team nursing is when the delivery is provided by a group lead by a professional nurse. The team divide the roles according and communication is fundamental. Primary nursing involves one nurse being responsible for supervising the total care of hospitalized clients for 24 hours a day, 7 days a week. Physical pain is more easily addressed by the administration of medication or a non-pharmaceutical intervention like repositioning, or the application of heat or cold. Nursing care on a general medical unit is about patient and family centered care which is in alignment with Kolcaba’s Theory of Comfort. A large portion of the patients seen on this type of unit have multiple comorbidities and challenging social situations that require assessment of their past health history, their support system, and their current living situation. All of this is taken into consideration in multidisciplinary rounds where data that is collected is communicated to all disciplines and a plan of care developed for each patient. The unit which I currently manage assembles our multidisciplinary unit daily. Needs are identified and assigned to the team members who include social work, care management and therapies in addition to the nurses and the providers. Since the team meets daily there is an opportunity to evaluate the effectiveness of the interventions prescribed. Good teamwork is important in a patient centred care. It is a team of health professionals who actively participate, cooperate, interact, communicate expertise, respect, trust and its main focus is to improve patient’s health (Miller, 2008, p.14). Also, the team includes the family of the client and the patient itself (Miller, 2008, p. 15). Therefore, all members have a role to play. For instance, in the nursing practice it involves health promotion and maintenance regarding patient’s health in order to decrease the impacts of negative outcomes (NMBA, 2010). Nevertheless, this can be maintained under the national competency standard (NMBA, 2010). Part of the national competency standard promotes professional responsibility, multidisciplinary approach, critical thinking and client care delivery (NMBA, 2010).
The World Health Organization (WHO) (2010) defines obesity and being overweight as an abnormal or excessive fat accumulation that may impair health. Body mass index (BMI) as Arterburn et al (2008) put forward is a measure of body fat based on height and weight and further suggest that any individual with a BMI of more than 30 is considered as obese. Tannahill et al (1999) advocate that health education could be viewed as communication activity aimed at enhancing positive health and preventing ill health in individuals and groups by influencing their beliefs, attitudes, and behaviour of persons with power and of the community. Butler et al (2008) defines health prevention as the measures for early detection of departure from good health. Whereas Heath (2002) concurs that the role and function of the nurse emphasise on health promotion and illness prevention, the Department of Health (2010) emphasises on the prevention of diseases, prolonging life and promoting health rather than treatment.
The Healthy People 2020 (Healthy People 2020, 2014) is a nationwide program created by the U.S. Department of Health and Human Services to achieve objectives for promoting health and preventing diseases. Faith based communities may benefit tremendously in including these objectives in program planning. The main goal is for the American people to have long, healthy lives. The Healthy People 2020 program provides measurable objectives that can be applied down to the local level such as in faith based communities. These guidelines encourage high quality of life by being healthy, disease-free, accessible healthcare, and have an environment that promotes good health and wellbeing in all age levels.
Healthy people 2020 can be defined as that arm of the government launched by the Department of Health and Human Services in December 2010 with 4 goals which include attaining high-quality, longer lives free of preventable disease, disability, injury, and premature death; achieving health equity, eliminating disparities, and improving the health of all groups of people; creating social and physical environments that promote good health for all people; and promoting the quality of life, healthy development, and healthy behaviors across all stages of life (CDC, 2015).
In order to increase patient satisfaction by providing a more efficient method of continuity of care, Clark and the staff nurses proposed an innovative care delivery model that placed a Patient Care Facilitator (PCF) in charge of about 12 patients each (Clark, 2011). She further explains that each PCF will head 2 Registered Nurses (RN) and a Certified Nursing Assistant (CNA) for the same group of patients (Clark, 2011). Staffing plays a key role in continuity of care by having the same nurses staffed to the same group of patients with the PCF available 24/7.

It acts as a national guide for disease prevention and health promotion. There are various objectives and goals which serve as the guideline to achieve overall improvement in health . ‘Educational and Community-Based Programs’ is one of them. According to U.S Department of Health and Human Services the main goal of these programs is to “Increase the quality, availability, and effectiveness of educational and community-based programs designed to prevent disease and injury, improve health, and enhance quality of life.” (HHS, Healthy People 2020, 2011). This particular goal of Healthy People 2020 focuses on increasing the number educational and community based programs to increase awareness about disease prevention and health among every individual of the society. It focuses on educating them on various topics including chronic diseases, substance abuse, prevention of injury and violence to encourage and enhance health. It emphasizes on increasing the quantity and quality of such programs so that a larger population could benefit from the program and gain effective knowledge to achieve a healthy life.

Due to the accomplishments that has been achieved in the previous years under this program, the Healthy People 2020 expanded or rather broadened their goals and objectives beyond disease prevention, health promotion and reducing health disparities, but to also eliminate healthcare disparities that has plagued this nation by the year 2020. The governing body, the United States Department of Health and Human Services is responsible for coordinating this program; track the progress of these objectives so as to improve the health of the population. Among the various goals set by this body, only three will be discussed in this paper to gain more insight as to what it entails. These include health equity to improve the health of all groups; social and physical environments that promote good health; and to promote quality of life, healthy development and healthy behavior across life stages.

The prediction is that all health care providers will spend at least 50 percent of their time working with older adults; increased competency while eliminating ageist attitudes is paramount for quality health care (Ferrini & Ferrini, 2013, p. 15).

Healthy People 2020 covers 42 topic areas with approximately 600 objectives, which include 1,200 measures. A smaller set of Healthy People 2020 objectives, has been designated to communicate high-priority health issues and actions that can be taken to address them, this objectives are called Leading Health Indicators. The program goes above and beyond these health indicators in order to provide the best care for the people of this country. These indictors cover from the access of health service, nutrition, physical activity, and obesity to substance abuse, environmental quality, injury and violence.

The biomedical model of health focuses on the physical and biological aspects of disease and illness. It is a medical model of care practised by doctors and/or health professionals and is associated with the diagnosis, cure and treatment of disease. The Biomedical model of health has been evolving for many years leading to improvements in medical science, technology, increase in cures and treatments ie: increase in vaccinations /immunizations emphasis on diagnosing and treating individuals separately from their lifestyle/living conditions. This model of health concentrates on the disease, illness, or disability and attempts to (cure) return the physical health of the person to a pre-illness state.
