The new IOM report has put a lot of responsibilities on the healthcare industry to change its face from a sick to wellness approach. With this new demand many feel compelled to start a new era in the way of putting the patient at the center of healthcare. With new laws enacted and opportunities at hand nurses have an enormous role to fulfill. Influence by the expectation of the public and our devotion to our calling, great steps are taking to make sure that the patient is receiving excellent care not only in the hospital setting but across the country. Although I respect and trust nurses and doctors, I always carefully observe what is being done with myself or my family members. After watching Josie’s story and being in the process of becoming a medical assistant, I feel this story has given me an initiative to ensure patients and their families are kept safe. The generation we live in is technological, there are many resources for patients and families to utilize to educate themselves when it comes to medical conditions. Some people like to self-diagnose and it makes it harder for doctors and healthcare workers to work with those patients. Nurses should foster collaborative planning to provide safe, high-quality, patient-centered health care. As nurses, we probably know the patients the best since we spend much more time with them than any other members of the care team. We are the liaison between the patient and the rest of the care team. We need to identify a potential problem or issue, and bring it to the attention of the whole care team. We should facilitate mutual trust, respect, shared decision-making, and open communication among all relevant persons in the care of the patients. With the collaborative support of the whole care team, the patients are in a much better position and are more likely to make an informed and deliberate decision. In particular, the use of Florence Nightingale’s theory of nursing as the basis of my new practice had allowed me to begin with the basics and work my way forward toward a more modern and holistic approach to nursing care. While Florence’s work focused mainly on the military and her care of soldiers, she began to establish schools of nursing to promote nursing education and to encourage people to view nursing as a viable profession, (Alligood, 2014. 63). Nightingale’s focus may have been care of the military, but she made great strides on getting recognition for nursing and her theories still affect the practice of nursing today. In the article, Nurses as Leaders, the author proposes that nurse leaders have made great strides since Nightingale’s time when nurses were conscripted to help care for wounded soldiers. Such a setting required these early nurse pioneers to rely on critical thinking and quick action in order to save lives. The American Nurses Association (ANA) developed a foundation for which all nurses are expected to perform their basic duties in order to meet the needs of the society we serve. The ANA “has long been instrumental in the development of three foundational documents for professional nursing; its code of ethics, its scope and standards of practice, ands statement of social policy.” (ANA, 2010, p. 87) The ANA defined nursing as “the protection, promotion, and optimization of health and abilities, prevention of illness and injury, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations” and used to create the scope and standards of nursing practice. In closing, as the healthcare climate consistently changes across the United States, it is imperative that registered nurses understand the demand for superior knowledge and lifelong commitment to nursing. As nursing theory and practice expand, our role as leaders is to place a high priority on our commitment to educate and guide nurses, thus improving the lives of their patients through the support of transformational leadership. The Institute of Medicine’s recent report, The Future of Nursing – Leading Change, Advancing Health, specifically calls for nurses to advance and embrace leadership roles on all levels and fronts – from collaborative improvement to positive identification of areas facing challenges within the healthcare setting.
The article I chose discusses the continual change in the roles of nurses. The article also poses a concept that nursing now is not based on caring, but medicine. “By accepting continual changes to the role of the nurse, the core function of nursing has become obscured and, despite assuming medical tasks, the occupation continues to be seen in terms of a role that is subordinate to and dependent on medicine.” (Iley 2004) Nurses are taking a more professional role, and more tasks are being delegated to assertive personnel. Therefore, with all these changes occurring, the role of the enrolled nurse is unclear. “Previously, having two levels of qualified nurse in the United Kingdom had been seen as problematic for health service managers and nurses themselves, and the ending of enrolled nurse programs in 1992 helped to solve this problem.” (2004) The study in this article gathered the characteristics of enrolled nurses and differentiated the groups converting to registered nurses, groups in the process of conversion, and groups interested or not interested in conversion. This study reveals the situation of enrolled nurses in context of continuing towards the professionalization of nursing. “The data from this study support the possibility that the role of nurses as direct caregivers is seen as a positive dimension of the work they undertake.” (2004) The findings imply that nurses need to get back to being caregivers, instead of concentrating on obtaining professional status in medicine.
