In this paper the therapeutic relationships between clients and nurses in a recording are examined. This was done through exploring the client’s messages, defining what the messages mean, and then finally expressing the messages therapeutically help the client.

The therapeutic relationship between client and nurse is examined through the video of clients voicing their experiences with nurses. Key verbal and nonverbal messages about therapeutic behavior are found within the video and include intelligence, positivity, friendliness, encouragement, and compassion. These ways in which clients feel about the nurse all result from situational, social, and institutional factors.

During the time when all nurses were undervalued, Gordon followed and observed three registered nurses every day at Boston’s Beth Israel Hospital in Boston, Massachusetts, while on their daily routines for almost two years. Each of the nurses have different jobs, which cause them to have different roles. The three nurses Gordon evaluated were: Nancy Rumplik, an oncology nurse; Ellen Kitchen, a home care nurse practitioner; and Jeannie Chaisson, a clinical nurse specialist. All three nurses together have more than 50 years of work experiences in the medical field. Gordon gives us an assortment of cases the nurse worked on. The application of the therapeutic nurse-client relationship is absolutely essential to providing the appropriate care to clients (College of Nurses of Ontario, 1999, p. 3). Effective use of time is vital to the relationship, though many nurses believe they do not have enough time to provide care due to the current health care pressures and time limitations. A study by Forchuck, then analyzed by Hagerty and Patusky, was conducted between 10 nurse-client relationships; Forchuck reports that 7 out of the 10 established a “working relationship” between nurse and client within 2.5 and 6 weeks. Regardless of time, many nurses are introduced to a client with the presumptions that the client wants to receive care and create a relationship, though the nurse expects nothing in return. “Heifner (1993) found that nurses identified vulnerability of patients as an essential foundation of nurses’ abilities to ‘connect’ with patients” (Hagerty and Patusky, 2003, 146). Despite nurses building a relationship upon vulnerability, Hewison discovers that control can actually create “barriers to open and meaningful communication and exchange” (2003). Therapeutic relationships are an essential part of nursing; they are the foundation of nursing (CNO, 2009). The National Competency Standard for Registered Nurses states that nurses are responsible for “establishing, sustaining and concluding professional relationships with individuals/groups.” Throughout this essay, the importance of forming therapeutic relationships will be explained. The best way to understand exactly what a therapeutic nurse-patient relationship is is to look into the characteristics of the relationship. As previously stated, the therapeutic relationship focuses on the client and their goals. In contrast with a social relationship, the actions of both the nurse and patient are dedicated to the needs of the patient. Nurse and patient work together to attain the goals set for the patient. Although the nurse may enjoy interacting with the patient, this is not the purpose of the relationship. The parameters of the relationship are defined to ensure that the relationship does not curve in that direction. Parameters include, “[the] relationship is terminated when goals are met and service no longer needed” (Craven & Hirnle, 2009, p. 330). This ensures that boundaries are not crossed and the nurse-patient relationship stays a professional one.

The capability of an individual to understand and relate to the client is called empathy. When an individual is effective in empathizing with the client, it can be argued that a patient is likely to feel validated (Davies, 2014, p.198). This validation creates a sense of acceptance and closeness in the relationship between the nurse and client.
