Por Tu Familia Dos Reevaluation Paper

Por tu Familia Dos is an education based program and is a free for qualifying individuals. This program is 1 month in length and will continuously run, starting at the beginning of the month and ending at the end the month. The first 2 months of the program, will be used to hire and train staff and volunteers. The staff will consist of 1 registered nurses, 2 medical assistances, and 4 community health workers; program volunteers will consist of a minimum of 4 volunteers. All paid staff will be bilingual to be as culturally complete as possible.
Por tu Faimila Dos will run a long month pilot to learn what works and what does not work within the community. This will take one month to reevaluate the program. The reevaluation process will focus on time commitment, times and locations offered, and address the comments from the pilot study participants. The pilot will run just the same as the normal program, but will be followed by intense evaluation and modifications. After the program is up and running, for a minimum of 2 cycles (3 months per cycle) the program will be reevaluating again.

The time that program uses the community center will be donated by the local Parks and Recreation services. Individuals who wish to participate in the program will need to sign a medical wavier and consent form. Participants will be given physicals and medical clearance by the physician’s assistant students from Heritage University and will be instructed to go to the hospital if the blood glucose is ever above 400mg/dL. Qualifying individuals will be given a glucometer and 100 test strips from Walgreen’s, a water bottle with a $10 gift certificate inside provided by Fiesta Foods, and El restaurant will provide food and give out a coupon for $5 off when you spend $20 at the
