A posterior tooth which is grossly decayed, having periodontal problem, furcation involvement and fracture of root is difficult to restore.1 The treatment modalities for such tooth are very limited and depending upon root involvement they can be root amputation, hemisection and lastly extraction followed by replacement with partial denture or implant.2 It is well known fact that natural tooth is always superior to artificial one. After confirming working length with hand K file, canals were cleaned and shaped using NiTi protaper rotary files. Calcium hydroxide was placed as intracanal medicament for one week followed by obturation (Figure 2). After one week patient was recalled for hemisection. After administrating local anaesthesia full thickness flap was reflected to expose furcation area followed by separating the distal half of tooth by giving vertical facio-lingual cut till furcation area with help of carbide bur at high speed with air water as coolant. After complete separation distal portion of tooth was extracted (figure 3) .The empty socket was irrigated, cleaned and furcation area was smoothened to prevent any further periodontal problem. Flap was then repositioned, sutured and patient was recalled after one week and radiograph taken (figure 4). After healing post space prepared with respect to mesial root, post luted with resin cement followed by core build up and tooth preparation for full coverage crown (figure 5, 6). Final impression was made using putty impression material, master cast prepared and sent to laboratory for fabrication of porcelain fused to metal crown. So far, various techniques have been used for reconstruction and regeneration of maxillary and mandibular bone defects. Autogenous bone grafting, guided bone regeneration (GBR), distraction osteogenesis and nerve transpositioning are among these regenerative techniques (1-8). Decision making for the treatment could be influenced by the type, size and location of the bone defects (2, 3, 9, 10). GBR had high success rate in treating small alveolar defects such as dehiscence or fenestration. Regenerative bony walls around the defect with ingrowing blood vessels can begin osteogenesis (11) larger bone defects with insufficient regenerative walls and an low quality avascular bed need varied amount of autogeneous bone graft from extra oral or intra oral donor sites, however, the patient may suffer from complications in donor site as well as bone graft resorption.(10, 12-15)

The biggest shortcoming of these archaic appliances was the fact that they were subject to decay in the wearer’s mouth. Thus, installed false teeth would have to be replaced on a regular basis, resulting in a very costly procedure. However, this problem was rectified in 1774 by Duchateau and Dubois de Chemant with their invention of the first full set of dentures that would not rot. This was due to their porcelain composition – a material that was much more conducive to the everyday wear and tear of one’s teeth. Unable to produce anything less than a full set of teeth (the surrounding porcelain was required to keep each tooth in place), patients who were missing only one or two teeth were out of luck, unless they were willing to have the rest of their teeth removed as well. And yet, in 1808, Giuseppangelo Fonzi developed the first individual appliance – a single porcelain tooth that could be held in place by a pin drilled into the jawbone. Finally, in 1845, Claudius Ash, known as the official “inventor of dentures”, produced the porcelain version that is now used today. His contributions included a suction method of adhesion for a full set of dentures, so that no form of attachment could be seen by an onlooker.
