Grocery shopping is an existential item of life that everyone has to partake in, normally, on a regular basis. Even as the national age continues to increase with the ‘baby boomers’ beginning to edge into retirement and enter assisted living it does not change the fact that they are still in dire need to continue to shop. Within the clinical experience, it was interesting to attempt to be an elderly individual. This was different and really brought into light the complications associated with it, and being partially disabled. Concerns arose from the mobility complications present but were easily accommodated by the use of a motorized chair and the assistance from a select few other individuals. In the grocery shopping experience, there was a strict budget that needed to be followed. However, in doing so it again stressed the reality of the experience and proposed a level of difficulty. In the clinical setting, it became apparent as an elderly individual that it was difficult to obtain the needed items due to the mobility and functional level of the client. It was most surprising throughout the experience, that the items needed to be bought could all be gotten on a relatively tight budget; one just needed to graze the shelves for the cheaper price. A nurse who is advocating for their client needs to plan for or take into consideration assistive and transportation options available to the client. This is also information that is needed to be provided not only to the client but also to family members of concern. At the multidisciplinary meeting, the nurse will collect and assess the information provided by the other disciplines and family members stating that the patient is not at her prior level of functioning and then analyze the information to develop a diagnosis of deconditioning. Next, the nurse identifies outcomes for the patient to get stronger, achieve prior level of function, have activities of daily living (ADL’s) met in a safe environment by planning for home health, equipment, and 24/7 supervision through family or placement in a facility. This will be implemented by coordinating delivery of a walker and a 3 in 1 chair prior to discharge to daughter’s home with the home health agency nurse, physical therapist, and aide scheduled to start that day. In a week, the nurse evaluates that outcomes are being met by following up with patient, daughter, and home health agency evaluating that the patient is getting stronger, ADL’s are being met, and will soon be able to return to living independently. To achieve these standards of practice, every nurse should be aware of her own nurse practice act to ensure to be functioning with in the laws of the nurse’s state and to ensure the best outcomes and safety of the patients. For patients who are less affluent than the nurse, it is important to respectfully teach the patient about particular health care issues so they can be avoided in the future. Provide a nursing care plan that is comprehensive and easy to understand. For more affluent patients, it is important to maintain the same level of care the nurse would provide to anyone else. The affluence should not make the nurse feel less than or that he/she is not worthy of caring for that patient. It is important not to cater, spend more time, or provide better care for that patient and their family than the nurse normally would for their patient next

The act of caring for others has been recognised as one of the most important aspects of the nursing profession and labelled an “art.” Nursing encompasses autonomous and collaborative care of individuals or groups, sick or well and in all settings. In Nursing, there will always be instances where the patient's nurse needs to advocate for their patient. There are numerous reasons why a nurse would advocate for their patient ranging from getting the doctor to change the patient’s orders, helping the patient’s treatment team understand what it is the patient is requiring for the day, to expressing the patient’s last wishes before death. In every situation, the nurse should do what is in the patient’s best interest.
