On 08/18/2016 at approximately 9:30 PM, this Investigator conducted an interview with Janosha Fulwilder (DOB: 02-05-94) in her current living location located at 1457 W. University Drive Apt #67 Mesa, AZ. This Investigator advised Janosha of her rights and gave her a copy of DCS Notice to Inform. Janosha agreed to discuss the allegations in the report. According to Janosha, she has been living at her boyfriend 's mother 's apartment at 1457 W. University Drive Apt #67 on and off again and she only stays there with her boyfriend and daughter two to four days a week or every other week. Janosha stated that her daughter would say "where 's daddy" but means "where 's Vira" referring to one of the children that stay in the home. This Investigator …show more content…
Janosha was not aware of Saiyuka getting out of the apartment until Mesa Police were there attempting contact with them about Saiyuka getting out. When asked by this Investigator how Saiyuka was able to get out Janosha stated that she did not know because she was asleep. Janosha did not have anyone in place to watch Saiyuka because Saiyuka was also asleep when Janosha and Arien fell asleep. Janosha said that she thinks Saiyuka climbed up on the couch and opened the chain lock in order to get out. Janosha solution to the issue was to "maybe move the couch". This Investigator asked Janosha about the living condition of the apartment, and she did not seem to understand the issue. Janosha stated that the apartment is only "messy" because it has been an off two weeks and that school recently started. This Investigator noted that the living condition and trash in the apartment appeared to have been building up for a period of at least a few months. Janosha stated that she did not believe there was an issue with the living condition of the home and that there was not an issue with Saiyuka getting out of the apartment. This Investigator asked Janosha about the prior time Saiyuka was found wondering outside of the apartment without supervision on 08/05/2016 and Janosha stated that she was a school and was not watching Janosha at the time. However, it should be noted that this Investigator and law enforcement both had concerns over the living conditions of the home. When this Investigator entered the home at 1457 W. University Drive Apt #67 Mesa, AZ there was a strong smell of urine and body order. This Investigator observed trash on the floor, counters, sinks, hallways, living spaces, and bedrooms. This Investigator observed the trash containing old food, food containers, bags, soda cans, cat litter, cat feces, and other miscellaneous trash items. The apartment had stacks of dirty dishes in the kitchen sink and throughout the counters. This Investigator observed a large cat litter box in the main hallway that did not appear to have been cleaned for several weeks. This Investigator also observed clothing items in piles throughout the apartment. Several rooms in the apartment appeared to be cluttered with various items. This Investigator staffed the living conditions observed with supervisor who stated to advise them to clean the

DSS received a report of physical abuse, physical neglect and substantial risk of physical abuse on March 3, 2017 alleging there is chaos in the home. After returning with his brothers from their cousin’s house, the boys had to go to bed. The reporter stated that Xzavia banged on his mother’s bedroom door on three separate occasions wanting something to eat. The first time he was told to go back to bed and a popping noise was heard. The second time Ms. Kimberly Dawkins grabbed Xzavia by the head, shook him little and told him to go back to bed. The third time Ms. Dawkins grabbed Xzavia, threw him on the bed, got a gun, held him down and pointed it at him. Reporter states there was no clip in the gun. Ms. Dawkins was reported being upset and it was reported that she left the home for twenty minutes but came back. It was reported that while Ms. Dawkins was gone, Xzavia was very upset and he was crying and Tyzhaun stared punching holes in the wall. It was reported that Xzavia says he wanted to die and that Atmorris went and got a knife, stated to Xzavia, “You want to die?”, here’s the knife and the two older brothers, Tyzhaun and Atmorris left the house and Xzavia chased them. It was reported that Xzavia was outside crying and he spent the night with the neighbor.
CCIB Intake received referral #0277-5021-7355-6088130 dated 2/18/16 from reporting party Ignacio Gonzalez Lead Residential Counselor. It was reported that residents Terri'Aunna Unique Reliford DOB: 9/1/00 and Endia Keairra Wilson DOB: 9/6/01 went AWOL (date not provided). Resident Endia returned to the facility on 2/17/16 and stated that Terri'Aunna was with and 18 year old male named Roy Smaer IV. Endia reported that Roy is a "meth head" and lives in Long Beach. According to the reporting party Endia stated the last time she saw Terri'Aunna (date unknown) she appeared tired, had bags under her eyes, looked high and not bathed. The crime scene includes Merediths room were her body was found, the bathroom where blood and someones feces was left in the the toilet, and the one of there other roommates room had been broken into. Prosecutors and detectives have spoken out about this information. Accorrding to crime scene photos and descriptions, between all the rooms that were shared between multiple roommates, there was significant amounts of blood found in each room. In meredith’s room, Rudy Guede’s fingerprints were found everywhere, his fingerprints were even found in Meredith’s blood written on the wall, which he claimed to be trying to understand what Meredith was saying so that he could try to save her. Paradise Village Parkway, Phoenix, AZ for Case # 537001 report of Child Neglect for victim Fiona McFadden (DOB: 03-05-2013) against her mother Monica Katich (DOB: 09-17-89). Upon arriving on the scene, this Investigator met with the report source, Phoenix Police Officer E. Gomez # 7977 in reference to Phoenix Police DR: 2016-00001441605. According to Officer Gomez, Phoenix Police received an emergency 911 call from Monica Katich that her friend later identified as Ashley Brook Post (DOB: 06-21-83) had overdosed on Heroin. The room was fully furnished, including bed and closet space. Overall, adequate space was observed both inside and outside of the home. Furthermore, the home was safe. Working smoke detectors and a fire extinguisher were located in the home. The home is on about 20 acres of mostly grass and has a pond located in far front yard.
