1.) How would you begin to evaluate the need for the policy and the possible support or lack of support for the policy from your peers, nursing management, and others who might be affected by the policy? I would begin evaluating the needs for a health policy by talking to other staff members, observing patient care, and examining readmission rates. “Health policy refers to decisions, plans, and actions that are undertaken to achieve specific health care goals within a society” (Guido, 2014, p. 50). I really do believe the ED discussed in exercise 4-4 needs a health policy of some sort put into place. The nurses seem to be doing their in part making sure patients get a suitable discharge teaching because they know it is the ethical thing to do. Provision 2 states that “the nurses’s primary commitment is to the patient, whether an individual, family, group, community, or population” (American Nurses Association, 2015). The nurses at this facility are committed to their patients, but the nurse manager is concerned about wait times. What the nursing manager may not know is cutting back on discharge teaching could lead to serious and even deadly consequences for the patients who visit this ED. I believe the best thing to do is have the staff discuss …show more content…
with the nurse manager that decreasing discharge teaching time is dangerous. Instead of taking away patient education, maybe the facility could hire more nurses or a nurse assign one nurse each shift to be a designated discharge nurse. I believe making these easy changes could improve wait times and ensure that patients get plenty of discharge teaching. 2.) Do the 10 questions outlines by Malone (2005) assist in the process? The 10 questions definitely assist in the policy process. Using the questions stated can help address issues that may arise during and after the policy is completed. If the staff member in charge of making the policy asks themselves and other employees the 10 questions it can help to create a policy that will benefit all. 3.) Draft a proposed policy to address the issues you have identified. “Nurses are frequently involved in health policy issues in every day clinical settings” (Guido, 2014, p.
51). The discussed ED is having difficulties with long wait times in their facility. Both staff and patients are affected by this health care issue. Possible solutions that can be proposed are hiring more nurses or designating a discharge nurse for each shift. These suggested outcomes may need to be discussed with staff members and the hospitals administration before it can be implemented. Until changes can be made staff should continue to provide excellent discharge information and provide the best care possible for all patients, no matter the wait time. Staff should always remember that safe care is quality

Person centred care means basing the care and support of a person around them. Looking at things from their perspective, promoting their beliefs, preference, likes and dislikes. They are involved in the development of their support plans, risk assessments and what they want to achieve. They determine what they want and how they want things doing. It promotes their individual needs and what is important to them. We listen to the individual and find out about their wishes and look at ways of carrying this out as safely as possible. We work with the individual, their families and others to empower the individual and to promote independence in their lives and ensure that the individual is supported to maintain their lives as they
D1: I have decided to look at a 6 year old going through bereavement. Bereavement means to lose an individual very close to you. When children go through bereavement they are most likely to feel sad and upset about the person’s death. Children at a young age may not understand when a family member dies. Children may not understand bereavement. For example a 6 year old’s father been in a car crash and has died from that incident. Death is unpredictable and children can’t be prepared for a death of a family member as no one knows when someone is going to die or not. Unfortunately every child can experience bereavement even when a pet dies. It is important that we are aware that effects on the child so we can support them in the aftermath.
To enable a positive change in discharge time, the physicians would need to be engaged and accountable for their part in the initiative. Environmental Services would need to be educated on how bed turnaround time affects patient flow and they can help make this initiative successful. Social Services and Case Management would be integral in the process. Assuring that barriers to patient discharges are addressed early. In addition, nurses would need education on this initiative and positive effects that would be seen as a result of its success. Nurses will have to prioritize to assure their care and responsibilities do not cause delays. Improved teamwork between centralized bed placement staff and unit staff would help assure smooth flow of patients. Administration needs to continue to be highly visible throughout the initiation of this change and on a continued basis to sustain the positive effects of the change (White & Griffith,
What does ‘care’ mean? Care is the provision of what is necessary for your health, welfare and protection of someone or something. However when you talk about ‘care’ in a care practice the term changes and becomes more about enabling people to meet all their needs which would refer to their social, physical, emotional, cognitive and cultural needs. The individual is central to the meaning of care in this context.
stationed staff in one area of a hospital ED to treat patients presenting to the ED" (Minott, 2008). This results of this experiment stated that "28 percent of patients diverted back home and avoided hospital readmission" (Minott, 2008). Another study showed that "greater registered nurse hours spent on direct patient care were associated with decreased risk of hospital related death" (Kane, Shamiliyan, Mueller, Duval and Wilt, 2007). This shows that there is a correlation between direct care between healthcare providers and a better outcome amongst patients.
2.3 Explain how the health and social care practitioner own values, beliefs and experiences can influence delivery of care.

It is very important for the service user who receieve care to trust the staff who are assist them. In this way the service user will feel more confortable with the staff. As the baby boomers of the country come to an age on needing long term care, where does Medicaid and Medicare fall into helping with the expenditures these individuals may need?
I want to pursue a career in social work because I want to help people who are suffering from mental illness and who are underserviced when it comes to getting help for mental illnesses. By working with a distressed person to become better, not only will I be able to enhance their life, but also the community will transform into a more positive place. I view transformation as a ripple effect, where changing the situation for one person eventually causes a change in the majority. This is especially true when it comes to mental illness and the stigma that surrounds it. I have been highly involved in mental health work on my college campus since my sophomore year. I am one of the founding members of our Active Minds chapter, and have held the
As a young girl in West Africa, Liberia, seeing oppression and living through poverty developed my love for interventions and social services. My years of acclimating to American culture have enabled me to engage in my education and earn responsible positions such as a resident assistant (RA), and president of the University of Pittsburgh at Titusville’s Black Student Union organization. To effectively sustain my positions, I became accustom to wearing many hats on any given day and setting boundaries. These skills have helped me to prepare myself as a future social worker by allowing me to aide people outside the traditional consulting and medical settings while accepting the limits of what can be accomplished during a specified period
There are numerous public health problems that can be addressed in my Southside of Chicago community. Among the several public health problems facing my Southside of Chicago community there are two that are more urgent. Health education or one might say lack thereof is a problem that needs to be addressed. My community is plagued with many of the residents suffering from high blood pressure, diabetes, and the killer virus known as HIV. In most cases these conditions can be prevented with healthier lifestyles and access to nutritious organic foods. In addition, environmental health is another urgent problem my community is facing. Access to clean, safe water and air is supposed to be a fundamental human right aimed at a healthy environment. Yet, my community consists a waste contaminated beach, numerous deteriorated building that are still occupied, and a countless number of restaurant and stores supplying our residents with services that are endangering their health.

Across the country, many people are living with pre-existing conditions, living in food deserts, living below the poverty line and a long list of other factors that either hinders them from eating healthy or force them to eat healthily. When trying to live a healthy lifestyle in this country not only does the promotion of prevention matter, but also the promotion of Career and job opportunities matter just as much. In the United States, Money equals Power and money also equal the opportunity to create and live a healthier lifestyle.
