This week’s clinical experience has been unlike any other. I went onto the unit knowing that I needed to be more independent and found myself to be both scared and intimidated. However, having the patients I did made my first mother baby clinical an exciting experience. I was able to create connections between what I saw on the unit and the theory we learned in lectures. In addition, I was able to see tricks other nurses on the unit have when providing care, and where others went wrong. Being aware of this enabled me to see the areas of mother baby nursing I understood and areas I need to further research to become a better nurse. Luckily, I am very comfortable with patients, which made providing care very easy. I was not afraid to go into other patient’s rooms if they rang for help. I would try my best to help the patients; however, if I needed help I was not afraid to ask a fellow student or nurse on the staff. It is very important to be able to talk to patients and work as a team with fellow coworkers as it made the job easier. On the other hand, I need to go over mother and baby assessments to become more familiar with both. I was able to complete the assessments; I now need to do so in a timelier manner while ensuring I do not forget any key areas. Lastly, it is vital that I continue to go over patient teaching prior to clinical. Being comfortable with the patients made the teaching easier, though I need to become more familiar with all the material that needs to be taught. All things considered my first mother baby clinical went very well. I am more confident in the care I provided and will be providing throughout the next few weeks. If need be, the nurses were right there to answer questions or assist in my learning. I do know I have room for improvements, though I am aware it will take more practice. Having such cooperative patients and parents allowed me to take my time and be more thorough to ensure I was grasping concepts. As I continue to connect class content and what is seen in the hospital
I thought it was a very big challenge being a brand new nurse in the hospital. I would say that I struggled with confidence and overall stress for the first 6 months. I quickly realized that very few patients fit inside a clinical box that I had learned in textbooks. I was very task oriented, just from being overwhelmed with work load, and it was difficult for me to critically think in the way I knew I was capable of. A lot of this improved with time and supportive mentors. (5)
It was intimidating and a bit scary, but instinctively I tried to help the patient and his family in any manner I could. As the day progressed, I had less anxiety when administering medications to the patient, and I felt more at ease with checking on the patient and his family to ensure they had no unmet needs. Because of our initial encounter with the doorway assessment, providing patient care was not as frightening as past first days of clinical have been. As a result, I always felt that I am actively participating in patients’ care. She allowed me to perform patient examinations most of the time and encouraged me to build up a good rapport with the patients. I think my past experience and medical knowledge was helped me lot during the history taking because I was able to go through history taking in a systematic manner and at the same time I could think of possible differential diagnosis. Furthermore, working in a medical clinic as a physician assistant also helped me a lot because one of the responsibilities delegated to me is taking patients history, however, this time it was different that I had to work out and actively think about a possible cause for patient concerns. Some things I need to work on are making sure I prioritize what should be done, and making sure I delegate any task that can be delegated, if I feel overwhelmed. As graduation comes near, I feel like I have learned so much more from being on this unit alone. I feel like I could take care of a couple of patients by myself, I improved on my medications, educating the patient, and providing adequate care for each and every patient. While on the unit, I got the chance to ask a variety of nurses different questions about their transition from new graduate to register nurse. I asked them why they choose this floor to work on , and a majority of them said because they precept on this floor and feel in love with neuro and the staff here. I also asked, how was your experiencing transferring from school to your first RN job, one nurse said that she only had the opportunity to orient for about three months before they put me on the floor because of a shortage of nurses. She did say that it was an excellent experience and she had a chance to learn so much. This reflective essay will discuss three skills that I have leant and developed during my placement. The three skills that I will be discussing in this essay are bed-bath, observing a corpse being prepared for mortuary and putting canulla and taking it out. These skills will be discussed in this essay using (Gibb’s, 1988) model. I have chosen to use Gibb’s model because I find this model easier to use and understand to guide me through my reflection process. Moreover, this model will be useful in breaking the new skills that I have developed into a way that I can understand. This model will also enable me to turn my experiences into knowledge that I can refer to in the future when facing same or similar situations. Gibbs model seems to be straightforward compared to the other model which is why I have also chosen it. On my second clinical day I was able to embody the educator role, and be an active participant in health teaching with my first mother and baby assignment. The mother I was caring for gave birth in the early hours of the morning, and had a successful feed following shortly after. Unfortunately after this during later feeds baby did not seem interested with the breast. As she voiced her frustration after several attempts I found myself empathizing for her because I could tell she was wondering why? I reassured her and listened to her concerns. As mentioned by Hunter and Arthur (2016), one of the main reasons I could maintain and improve my practice was due to clinical placements. During clinical placements we are frequently being assessed and taught in dealing with real situations. Feelings As learning is a lifelong process, I’m sure there are many things I need to improve and learn to better myself. However, the lessons I learned during the course as a nursing student shall always remain as a bedrock for my future development.
I was then introduced to a patient who was in isolation. Her legs were immovable and were crossed in a very uncomfortable position. I wish I could’ve done something so that her legs could be in a more comfortable position, but all I could do was observe and get her a cup of ice cold water to drink. During this clinical observation, I didn’t get to see much but overall, it was a good experience. It made me realize what it was like to be in a hospital setting and what it meant to be a nurse. Seeing how the patients were still able to smile through all the pain they went through, it made me want to become a nurse even more because I would also like to make my patients happy. Clinical placements allow student nurses to put theory into practice with real patients and actual clinical situations. Every placement will challenge student, improve communication skills, build own knowledge and foster own reflective practice. It is one of the most interesting and exciting aspects of training to be a nurse. I personally believe that, students must be responsible for their own learning opportunities and understand the outcomes they need to achieve on placement, discussing any queries with their clinical supervisor. I always look for learning opportunities. I approach my clinical supervisor for her guidance to perform any new skills fall into my scope of practice.
I believe placing student nurses in the clinical setting is vital in becoming competent nurses. Every experience the student experiences during their placement has an educative nature therefore, it is important for the students to take some time to reflect on these experiences. A specific situation that stood out to me from my clinical experience was that; I didn’t realize I had ignored the patient’s pain until I was later asked by the nurse if the patient was in any pain.
New nurses are faced with discomfort as soon as they enter the profession. Hence, it is suggested that precautions be taken even before nurses graduate. The educational system should consider the involvement of clinical staff in the teaching process. This will enable student nurses to meet actual nurses prior to entering the profession (Goodare, 2015). In other words, to make the transition easy, nursing education must include actual exposure to the clinical practice where student nurses are required to not merely become observers but actually participate in the process of providing care. However, nurses in the maternity ward have expressed their deepest concerns about meeting the needs of first time mother on discharge education during their stay in the hospital.
