Purpose and Use of an Intake Form
Services are in need and adults, parents, children, and family come in and seek out the services of a social worker on a routine basis. There are enormous variety of reasons that a client would want to come in and seek treatment or assistance from a social worker. Many times, a future client or a current client is not really aware of what services or assistance they need from a social worker. The social worker needs to have some type of method to help understand the situation with a client. an intake form is provided to the client, which the client then reviews it and fills out the information to the extent that they can and are willing to provide.
