Community Agencies for Vulnerable Populations
Cabell Huntington Hospital, located in Huntington, West Virginia is a private non-profit facility with a mission to meet the lifetime health care needs of the community (CHH, 2015). The hospital is committed to the older population in the area, developing a Senior Services Department in 2003. In 2006, the hospital became a designated Nurses Improving Care for Healthsystem Elders (NICHE) facility (CHH, 2015). Along with their inpatient program, the hospital also developed a senior outreach program, which will be the focus of this paper. The goal of the outreach program is preventive care and health promotion for the vulnerable population of older adults (CHH, 2015). This program strives to meet the goals of the Healthy People 2020 initiative of attaining longer lives, free of preventable disease, disability, or injury, while eliminating disparities, promote health and quality of life (ODPHP, 2015).
Community Health Agency Description
The program’s preventive health and educational services include health screenings, vision as well as hearing exams, and biannual medication reviews by a pharmacist (CHH, 2015). Cabell Huntington Hospital’s (2015) outreach program provides ongoing free exercise programs and social activities for seniors as well. A local television station and the hospital collaborate to provide daily broadcasts regarding health promotion and health topics targeting the senior population in the community (CHH, 2015). Their program publishes a monthly calendar with activities, such as Alzheimer’s and Parkinson’s support groups, Medicare information workshops, and education regarding diet and nutrition (CHH, 2015). In conclusion, Americans are living longer, and the senior population is growing in numbers, expected to reach 20% of the United States population by the year 2030 (Nies & McEwen, 2015). Health promotion and preventive services are key community health strategies to ensuring older adults maintain independence and functional capacity as long as possible (Nies & McEwen, 2015). Public health nurses must continue to emphasize collaboration among health care professionals that treat older adults to ensure access to health care for all, including those in hard to reach rural areas (Nies & McEwen, 2015).
