Public health nursing (PHN) is the predominant nursing practice in the United States. PHNs work in a several capacities in the community. The practice varies from the nurse work that the public commonly sees. Recently, researchers have developed a special work methodology for the nurses to use. Using the method, public health nurses have tackled community health issues with positive patient outcomes. PHN regulatory groups predict that these nurses specialists will continue to serve as a valuable asset for years to come.

This problem is especially pervasive in rural Southern states that grow the tobacco. Based on that report, a PHN group – called Healthy People – made recommendations to rural anti-drug organizations to curtail the trend. In addition to posing a long-term health risk to the youth, tobacco is also a gateway drug to other more harmful drugs such as marijuana and alcohol. While substance abuse programs are common in major cities, rural communities had little experience or knowledge dealing with the issue. The group makes sure that PHNs have the required skillsets to lead a community caregiving effort. The COL emphasizes that public health nurses must have a strong ability to marshal community leaders together to work on their community’s health concerns. Additionally, the COL defines public health nurses as “advocates, collaborators, educators, partners, policy-makers and researchers.” Moving forward, the COL urges public health nurses to increase the public’s participation in their health maintenance and encourages PHNs to expand on ethnically-based program creation to resolve community health issues. Nurse practitioners (NPs), one type of advanced practice nurses, are licensed by the states where they practice and certified by private boards. Nurse practitioners hold advanced degrees in clinical practice and function in a wide variety of settings and across the life span. They provide a broad array of healthcare services ranging from managing treatment plans, to prescribing medications, to implementing health promotion services. As of 2014, 205,000 NPs were licensed in the United States with 86% of those prepared to deliver care to patients in primary care settings (NP Facts, 2015). Provision 8 states, “The nurse collaborates with other health professionals and the public in promoting community, national, and international efforts to meet health needs.” (Nursing World, 2001). Provision 8 taught me that ANA’s goals are not limited to the health and welfare of individuals, but their goals also encompass the welfare of the community through promotion of collaborative work of nurses with other health professionals and the members of the community.
According to Allender, Rector, and Warner (2014), public health is a combination of both an art and a science (2014). The mission of public health nursing is to promote health, prevent disease and ultimately prolong life (Allender et al., 2014). In order for this to occur an assessment must take place. An aggregate or community assessment begins with a collection of data. This includes: the community’s health needs, risks, environmental conditions, financial resources through local census data, and a windshield survey (Allender et al., 2014). Through public health nursing, communities can collectively come together to help promote an overall better health standing.
Nursing is a knowledge-based profession within the health care sector that focuses on the overall care of individuals. According to The American College of Nurse Practitioners (ACNP), “defines nurse practitioners as registered nursed who have received graduate-leveling nursing education and clinical training, which enables them to provide a wide range of preventative and acute health care services to individuals of all ages. They deliver high-quality, cost effective care, often performing physical examinations, ordering tests, making diagnoses, and prescribing and managing medication and therapies”. Nurse Practitioners are able to specialize in a particular area, such as family and adult practice, pediatrics, and women’s health; and refer patients to other specialist when necessary. Some Nurse practitioners work under the supervision of a physician; while others run their own practices.
Educational preparation for the advanced practice public health nurse includes at least a Master’s degree and is based on an integration of current knowledge and research in public health, nursing, and other scientific preparations. In addition to executing the functions of the generalist in population-focused nursing, the specialist retains clinical experience in political and legislative activities, delivering and evaluating service, organizing, interdisciplinary planning, community empowerment, and assuming a leadership role in interventions that have a positive effect on the health of the community. Certification examinations for advanced practice nurses are rewarded by the American Nurses Credentialing Center (ANCC).
There is a shortage of all health care professions throughout the United States. One shortage in particular that society should be very concerned about is the shortage of Registered Nurses. Registered Nurses make up the single largest healthcare profession in the United States. A registered nurse is a vital healthcare professional that has earned a two or four year degree and has the upper-most responsibility in providing direct patient care and staff management in a hospital or other treatment facilities (Registered Nurse (RN) Degree and Career Overview., 2009). This shortage issue is imperative because RN's affect everyone sometime in their lifetime. Nurses serve groups, families and individuals to foster health and prevent disease.
Healthcare is a continuous emerging industry across the world. With our ever changing life styles and the increased levels of pollution across the world more and more people are suffering from various health issues. Nursing is an extremely diverse profession and among the highest educated with several levels ranging from a licensed practical nurse (LPN) to a registered nurse (RN) on up to a Doctorate in Nursing. Diane Viens (2003) states that ‘The NP is a critical member of the workforce to assume the leadership roles within practice, education, research, health systems, and health policy’.
The health care system of today will be invaluable without the help of well-trained registered professional nurses. According to Wilkinson, Treas, Barnett, Smith (2016) registered nurses in today’s health care have the training required to develop a care plan with other health care workers that is specific for each patient in their care, instead of the same general approach for every patient. This is known as patient centered care because each patient is different from the other and the registered nurse role includes developing a good plan for every patient they care for. In performing such duties, registered nurses tend to create a strong bond with their patients by caring for their patients by all means possible. By caring for their patients, they tend to gain their patients and their families trust. According to Wilkinson et al. (2016) There are many support systems for patients in our communities that have good intentions of assisting sick people and those in need. Most of them are not known by the patient or their families. The registered nurse is the one who is in the position to inform the patient or family about such resources that may be very helpful for them if they are interested. By doing so, the patient knows that the nurse really cares about him/her and that feels very comforting and may help the patient get well
During one of my undergraduate courses, “Community and Population Health”, I completed a paper on my community and access to healthcare. During the research and community outreach performed to meet the goals of the paper, my eyes were opened to the plight of small communities in regards to access to quality healthcare. This plight has become my passion, and has formed the basis for my vision of the Family Nurse Practitioner role.
“A public health nurse is a nurse who works to promote and protect the health of an entire population” (Allender, Rector, & Warner, 2010, p. 839) the public health nurse I chose to interview was Barbara, she currently works for the Health department and has been there for three years. During the interview Barbara discussed the requirements for a public health nurse. The individual must have the ability to exercise independent judgment, planning and able to administer safe effective public nursing care. In addition be able to work effectively with individuals and families to assist them in the satisfactory solution of health problems, by recognizing and respecting of different cultural, ethnic and difference on health care practices. The nurse is also required to know disease prevention such as, different techniques on teaching, counseling, interviewing and resources available. Barbara stated that the most important part of being a public health nurse is being able to care. Barbara acknowledged that there are numerous individuals that she assists everyday that just want someone to exhibit that they care and understand. The education a public health nurse should obtain is a bachelor’s degree in nursing, which Barbara posses. “Public health nurses working with specific populations or in administration position should hold a master’s degree” ( Allender et al., 2010, p.839). Barbara discussed that before becoming a public health nurse she worked at St. Francis Hospital as med-surge nurse for five years, after that it was imperative that she change working environment. Barbara confirmed that working as a nurse in the hospital was extremely diverse then working as public health nurse. “Comparing public health nurse and general nurse, public health nursing is population based, focuses on the greater good, health promotion and disease prevention, and utilizes community resources. The general nursing is individual based, focuses on individual good, restoration of health and function, and manage resources at hand” (Minnesota Department of Health, 2007). In the beginning Barbara thought that public health nursing would be a difficult job to gain knowledge of however, she established that it was exceptionally effortless and gratifying career to posses. Barbara stated that she did not realize how much assistance was out there for the public until; she worked as public health nurse. Barbara spends 95% of her day going from place to place by doing home visits, investigating different cases, assisting with screening programs and enormous amount of paper work.

Being a registered nurse affords one the option of working in many diverse healthcare settings. In any practice setting the climate of health care change is evident. There are diverse entities involved in the implementation and recommendation of these practice changes. These are led by the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM), nursing campaign for action initiatives, as well as individual state-based action coalitions. Nurses need to be prepared and cognizant of the transformations occurring in health care settings as well as the plans that put them at the forefront of the future.
To answer my questions I looked to my clinical instructor and other resources such as course literature. From the course book, I was able to learn that public health nursing can be applied to lead collaborative efforts to attain optimal health for people locally and nationally. It also helps develop innovative programs to improve the health of populations and lead health care reform efforts within and across health care systems. As far as working conditions, I have learned that most public health nurses work for government agencies, nonprofit groups, community health centers, and other organizations that aim to improve health at the community level.
