Among the issues or challenges that were identified on Staffordshire NHS Foundation Trust Public Inquiry I would like to focus on “high degree of tolerance of poor standards of care and tolerance of risk to patients”. Patients and visitors have the right to receive quality health care free from risks and violence. Health personnel and leader must be able to identify, assess and eliminate risks, errors, negligence towards patients care. They have to provide an appropriate response and action where error or violence occurs. Poor standard care should not be neglected nor tolerated instead need to be proactively identified, assessed, report and addressed which leads quality care and patients satisfaction (Victorian Government Initiative 2015). Risk management activities must review time to time by leaders and externally methods to improve patient safety and quality care. There must be access to information necessary for risk management process. Organizational culture must have open communication where leaders can support the victim of violence in the workplace, able to report incidents of error and negligence and manage accordingly which results patients’ safety and satisfaction (Ministry of …show more content…

(2017) states that effective leadership is vital for increasing patient satisfaction, provision of quality & standard healthcare, reduction in harm and risk to patients which aids to gain successful outcome in an organization. So this issue make alert to every leader in health organizations to give priority on the standard quality of care and always prevent patients from any harmful risk or error. They must focus on safe and quality care to the consumers.
