- At the beginning of the movie, Cobb and Arthur practice the method of "extraction", which you steal ideas from someone's dream and escape by merely killing yourself or get a "kick" to wake up from that dream. This was just one to two levels of dreams with little use of sleeping sedatives, which reduces the chances of being in limbo. - The reason that I explain this method of "extraction" is because is easier to perform than "Inception", which you plant an idea in someone else's dream. This is where the limbo part plays a key role. Yusuf, the chemist for the team, created a stronger sedatives for the team to last longer in each layer of the dream. I think his dream started when he try the new sedatives from Yusuf and he wash his face. He use his "totem" to determine if this was still real but before the totem can tilt down, Eames interrupted him by asking him if he was OK. The totem fell down but I felt like the opportunity for the totem to fall for itself never happen, which meant the dream started there. You can disagree with that theory, but that's how I interpreted of how his dreams started (that and of course the start of the mission is where his dreams started is also my other theory too). I think when he was with the old guy, he was still in obvious limbo, trying to kill Saito to wake him up. However, we never see Cobb waking up to kill himself from his memory, level 3, level 2 and level 1 of his dreams, but he still manage to wake up "miraculously" in reality with the rest of the gang, including Saito (I explain earlier that Cobb knew how to wake up in limbo and Eames has stated that Cobb can pull it off after they woke up from the second dream). When he spun his totem and saw his children faces for the first time, we never get to see the totem fall down when the movie ends. I assume that he was still stuck in limbo, just like his late wife always wanted him to be, but the totem was wobbling and starting to fall down. It lead me to think that he was back in reality all along, but I could be wrong. There really is no wrong answer for the conclusion of the film because there's only two conclusion that people can come up with and Christopher Nolan won't tell you the real
