Basic Information Identifying Information. Juanita Mouton, a Caucasian 24-year old single woman form Houston, TX, she has been living in Cypress, TX for the last three years. She is currently living with a close friend and her son, in Langham Creek Estates. She describes the neighborhood as quiet, near shopping venues, restaurants and education opportunities. Presenting Problem. Juanita is a mother of five children; her oldest Jacoby, who is 6 years of age, currently lives with her mother, his maternal grandmother. While she technically has legal custody, her mother has helped raise Jacoby since birth. She had Jacoby at the age of 17, while still in high school, and living with her mother. Juanita moved out of the home when Jacoby was 2 years of age, and moved back in and out several times over the course of Jacobys’ life. Juanita feels her relationship with Jacoby is “broken”. She reports he shows a lack of respect for her as his mother or an adult in authority. She indicates, when she tries discipline of any kind, her mother steps in. While she does not live in the home with him currently, she visits often, and has witnessed poor behavior which she feels needs to be corrected. She has stated, for example, when she does discipline him for shouting an expletive in anger, and she grounds him from his I-Pad for a week, her mother will not follow thru on holding him accountable for his actions.
