The patient presented with a shoulder disorder, a common orthopedic condition. To diagnose and treat the patient, the pathoanatomic diagnosis and the treatment based classification scheme called as staged approach for rehabilitation classification (STAR- Shoulder) was used as given by McClure and Michener 1. This classification is a staged classification and has three different levels: Screening, Pathoanatomic diagnosis, and A rehabilitation classification.
Level 1: Screening
On History- The patient was a 49-year-old Caucasian male with a chief complaint of pain and weakness in R shoulder abduction and external rotation (dominant shoulder). He was a retired baseball player. He has been a baseball pitcher for 12 years before he retired 5 years back. However, the patient still plays baseball as a recreation sport. Currently, he works as a cashier at Walmart. His job duties include the constant use of arms and sometimes heavy labor like lifting boxes and stocking them in shelves above the shoulder height. The pain was first experienced one month back while throwing the ball during the early cocking phase of a baseball game. The pain reported is 6/10 on the visual analog scale which is a deep dull aching pain on the lateral side of R shoulder which aggravates with movements involving overhead

The patient was also negative for any yellow flags.
On Physical Examination- On active movements, R shoulder abduction and external rotation were weak 3+/5 as measured by the manual muscle testing. On passive movements, the pain was reported with R shoulder abduction over 90 degrees and external rotation. Eccentric movements were more painful than PROM. On palpation, tenderness was present over the anterolateral aspect of the R shoulder. Sensations and reflexes were intact both Upper Extremity. At times the patient has night pain and rest

Rotator cuff tears are classified as either partial thickness or full-thickness tears, with the full-thickness tears being more severe.9 Before treatment occurs, the characteristic of the tear needs to be evaluated. First, the tear size can be evaluated by measuring the maximum diameter and cross-sectional area of the involved region of the tendon. 9 As described by DeOrio and Cofield, tears can be classified as small (one cm.), medium (one to three cm.), large (three to five cm.), and massive (more than five cm.). 9 Second, rotator cuff tears are evaluated based on the tear shape. 9 The first shape is a crescent shaped tear, which involves the insertions of the tendons torn from the head of the humerus. 9 To repair this, the tendon insertions must be reattached to the head of the humerus. 9 The second shape of tear is the U-shaped or L-shaped, which is the tearing between tendons and tearing between tendon and bone. 9 To repair this tear, the tearing of tendon to tendon is sutured, and the tearing of tendon to bone is fixed by the reattachment of the tendon to the bone. 9 The last parameter of assessing rotator cuff tears is tissue quality. 9 Muscle, bones, and tendons are all assessed through MRI for fatty infiltration/degeneration and for muscle atrophy. 9
Repetitive arm use can cause fatigue of shoulder muscles and this has been a potential link to the development of shoulder pain. A reduction in force generation of shoulder muscles might lead to a reduction in control or stabilization for joint motions, such as the GH joint. The infraspinatus has been seen to play a significant role in the alteration of GH kinematics when fatigued while other shoulder musculature, such as the anterior/posterior deltoid and serratus anterior, did not alter GH kinematics while fatigued. By having physiotherapy this will help to achieve and restore movement and function to the injured area of the body. To allow movement and function to occur an individual would complete a range of different exercise which, their therapist will show them how to do. This overall, would improve the ranges of movements and by improving the movements overall, it will help to heal the injured area so that, an individual would be able to do normal movements once again. This article is based on astronomy, which is very interesting to me making me excited to have found it. The article is about the “most mysterious star in the universe,” Tabby’s Star or KIC 8462852, which is a star that has recently become dimmer and brighter at random. The star is about 1,000 light years away, a little hotter, and a little larger than our Sun. This article discusses that the team of 200 scientists from Penn State and Louisiana State University are now getting closer to discovering why this phenomenon is occurring. In the video Star Size Comparison it displays the planets and stars all lined up next to each and it goes one by one from the moon to Mercury to Venus and Mars, Jupiter to Saturn etc.. I like the side to side comparison of all the planets/stars because it's a good frame of reference for how big the planets and stars are, in comparison to each other. The second video, To Scale the Solar System was a little different, in this video it primarily showcased the distance between the planets. A young man and his crew wanted to give you a sense of just how far the moon is from the earth and how far the Earth is away from Jupiter and Jupiter from Neptune and so on and so forth.
