Not only chronic diseases management becomes important at the heath care work but also it causes a state of challenge in many countries. And because some of the chronic diseases have turned into complex cases, they need multi-disciplinary team care.
By practicing within a team, healthcare profession¬als will be able to balance the amount of duties, responsibility and the workload. The term Team Care Arrangement TCA was first introduced to support multidisciplinary care for patients with chronic diseases. Building this type of teams may face many challenges such as poor communication and limited exchange of information. My team is responsible for the overall patient care plan and each member including the patient himself has his own accurate duties which include taking the right decisions according to the situation.
Although the team link intervention was based on inter-professional care, bringing professionals to work together doesn’t always guarantee collaboration with its concepts sharing, partnership, interdependency and power. So, building multidisciplinary teams always needs its leader to provide his members with clear objectives, specific roles and responsibilities, a pre -designed mechanism for exchanging information and coordination. At the same time, the leader’s role is to support his member with authentic resources and tools such as guidelines, structured protocols or policies and standards for communication to help them achieving their goals.

... effective collaborative work. There are many elements that may influence the relationship between the team members such as trust, respect, and diversity of opinions, mindfulness or openness to new ways of working, interrelatedness, social interaction and effective communication.
To have effective team, my leadership must be flexible, should have the willingness to innovate, being careful when choosing my team members and being careful when establishing the team. That of course will make my job easier and more professional. Using a mixture between the formal and informal communication skills is required. I must provide my team members with specified and clear roles and tasks and objectives. As a team leader; training and educating my team is my own responsibility. To make the job more achievable, I need to facilitate and organize the protocols and processes.
