Not only chronic diseases management becomes important at the heath care work but also it causes a state of challenge in many countries. And because some of the chronic diseases have turned into complex cases, they need multi-disciplinary team care.
By practicing within a team, healthcare profession¬als will be able to balance the amount of duties, responsibility and the workload. The term Team Care Arrangement TCA was first introduced to support multidisciplinary care for patients with chronic diseases. Building this type of teams may face many challenges such as poor communication and limited exchange of information. My team is responsible for the overall patient care plan and each member including the patient himself has his own accurate duties which include taking the right decisions according to the situation.
Although the team link intervention was based on inter-professional care, bringing professionals to work together doesn’t always guarantee collaboration with its concepts sharing, partnership, interdependency and power. So, building multidisciplinary teams always needs its leader to provide his members with clear objectives, specific roles and responsibilities, a pre -designed mechanism for exchanging information and coordination. At the same time, the leader’s role is to support his member with authentic resources and tools such as guidelines, structured protocols or policies and standards for communication to help them achieving their goals.

To have effective team, my leadership must be flexible, should have the willingness to innovate, being careful when choosing my team members and being careful when establishing the team. That of course will make my job easier and more professional. Using a mixture between the formal and informal communication skills is required. I must provide my team members with specified and clear roles and tasks and objectives. As a team leader; training and educating my team is my own responsibility. To make the job more achievable, I need to facilitate and organize the protocols and processes.
In this 21st century, there are more and more policies and guidelines that focused on long term conditions as these conditions are incurable but only can be controlled and progressed with long term management. In Northern Ireland, a policy framework “Living with Long Term Conditions” had been introduced and addressed about long term conditions (LTC) that needs high quality of care. This policy provides a better outcome with supporting good practice through 6 key development areas. The 6 key principles are essential in helping people with LTC to receive a better care, treatment and support. First area is partnership between the service user and the collaboration team whereby communication skill is highlighted to encourage service users to play an active role in managing their own conditions with individual care plan. Next, self-management is also another key principle to be developed so that those people with LTC managed their condition effectively which may progress over time. Through training and education on acquired skills is a good start to promote self-management strategy as they able to deal with flare-ups, condition and lifestyle. Thus, information is vital in helping them understand their own condition and knowing what is the best for them in order to increase their quality of life. A medicines management service help in bringing the best outcome for LTC patients while carer also need to maintain their own health to continue their caring role and act as a safeguarding through the provision of the support. Last principle is improving care and services at the right time and right way to prevent readmission and prolong hospital stay (DHSSPS, 2012).
In addition to having a specific objective assign and a clear role every team member should have the proper education that allows her to complete the assign task(s). Also an open and constant communication within team members is important, since the feedback from each other can help to the growth of knowledge of each of the team members as an individual, as well as it helps to resolve any issues or errors found within the interdisciplinary care model. Effectively coordinated and collaborative inter-professional teams are essential to the care and treatment of patients (Rowlands & Callen, 2013; Doyle, 2008; Ruhstaller, Roe, Thürlimann & Nicoll, 2006; Simpson & Patton, 2012, p. 300). Communication is a process of conferring information between individuals through use of speech, writing or various other means, and is critical to the success of a multidisciplinary team (MDT) (Higgs, McAllister & Sefton, 2012, p. 5; Rowlands & Callen, 2013; Sargeant, Loney & Murphy, 2008). An MDT must use multiple strategies to enhance communication and ensure their success (Doyle, 2008). An effective MDT generates opportunities that benefit healthcare, which is the reason for the recent dominance of inter-professional care in health practice (Simpson & Patton, 2012, p. 300; Rowlands & Callen, 2013). Many barriers prevent effective communication within inter-professional teams. Lack of communication within MDTs presents challenges to their success, leading to numerous consequences, including the failure of the MDT (London Deanery, 2012; Sargeant et al, 2008). Communication between professionals is the key factor underpinning the potential success or failure of inter-professional teams, the outcome of the functioning of MDTs will either benefit or impair care of patients.
The practice of using inter-professional teams in delivering care is not a new concept but current health policy requires professionals work within a multidisciplinary team Department of Health (2001) and entrenched in the Nursing and Midwifery Council (2008) Code. The principle focus of this essay is to discuss the importance of inter-professional collaboration in delivering effective health care and what challenges and constraints exist. The integration of a case study will give an insight into inter-professional collaboration in practice.
When they have been used correctly, they’ve been shown to improve efficiency and profitability in organizations. “Developing team skills is important because of the tremendous explosion in the use of teams in work organizations over the last decade (Developing Management Skills).” An examination of these models will show the similarities and differences they have in the context of team building. Four Stages Model “The Forming – Norming – Storming – Performing model of team development was first proposed by Bruce Tuckman in 1965. It is important that all team members are aware of their role within the group and that they are performed correctly to meet their goal. For example, at a nursing home a nurse and a CNA work closely together. As the CNA is preforming ADLs he/she is able to assess the patient for any problems that the resident may be having. The nurse is then made aware of any problems and may ask the CNA to assist in a situation such as turning the resident in order to administer a shot. By working together the medical staff are able to reach set health goals and help the patient more efficiently.
Chronic illness issues can include managing their illness, the cost of taking care of the illness, etc. Many people who suffer from a chronic illness suffer a lot trying to manage their illness on a daily basis. According to a website called NCOA.org, “About 80% of older adults have one chronic disease. 68.4% of Medicare beneficiaries have two or more chronic diseases and 36.4% have four or more. Chronic diseases can affect a person’s ability to perform important activities, restricting their engagement in life and their enjoyment of family and friends”
Intro- Collaboration with the interdisciplinary team plays a big part in the care of a patient.
Seven tasks must be included in consideration of team dynamics and structure. The first of which is defining the goal, mission or function of a specific team. The team must know what it is being asked to accomplish. The second area of consideration is assessing what skills, abilities, knowledge or potential to acquire such would be needed amongst selected team members. Identification of potential team members should include an assessment of the skills, knowledge and abilities or the potential to acquire such so that ultimately the team has the building blocks with which to succeed in its mission, goal or function. This assessment must include an understanding of realistic potential contributions by potential team members with the included assessment of whether or not the acquisition of skills and knowledge can be made available through research and analysis.

Interdisciplinary team work is a complex process which involves various disciplines within the health care setting which work together to share expertise, knowledge and skills to impact patient care. However interdisciplinary team work is increasing as a result of an aging population and higher number of patients with more complex needs associated with chronic diseases; along with increasing complexity of skills and increasing specialization within health professions (Susan A. Nancarrow, 2013).
The first stage in developing a team as it relates to group dynamics is forming, this is the stage where team members come together and they question what they are therefore, who else is a part of the team, who they are comfortable with, and this enables them to get involved as well as allowing the team the opportunity to introduce themselves to each other. The second stage is storming, and in this stage the team members begin to voice their opinions and differences as well as align themselves with others who share their same beliefs. This is an important stage for the team because team members will begin to become more involved with one another, and when they voice their concerns, they can feel like they are being represented and understood. The third stage is norming in the stage team members begin to establish a shared common commitment to the purpose of why the team is there, where they can establish their overall goals and how the goals are going to be achieved. In the fourth stage, known as performing, the team works effectively and efficiently to gather towards achieving the goal.
Another crucial thing that I learned from the EPP is the essential importance of teamwork in leadership and the various ways through which effective teamwork can be promoted. A team is simply a group of people who work together, cooperate with, and depend on each other towards the attainment of common goals and objectives. As a leader, it is important to possess adequate teamwork skills and knowledge. Teamwork is an integral component of the success of any given organization and so it is critical for leaders to possess the requisite knowhow to facilitate proper and effective teamwork within their spheres
