The LPN-Team Lead contacted the social worker about Dr. Sundaram’s patient. The patient is a single, Caucasian grandmother and mother of two; she is alert and orientated to person, place and time. The patient reports that she lives with her 16 year old daughter and 3 month old granddaughter. The patient states that she works two jobs, one full-time and one part-time job and she assist with the care of her new granddaughter while her daughter is a work. The patient report that she is feeling (angry) and hurt because her boyfriend of 11 years cheated on her when she was in the hospital and left her a month ago; this and the loss of her child last year at 6 months gestation in addition to her CHF, COPD and influenza appears to have left the patient feeling of depression and hopelessness. The social worker noted that the patient scored a 19 on her PHQ-9, although she denies thoughts of suicide at this time. Sundaram to administer medication for the insomnia and depression, enough to last 6 – 8 weeks as the patient is not likely to get an appointment with the psychiatrist at Wellstone Behavioral Health for up to 6 – 8 weeks. Dr. Sundaram prescribed 30 10 mg of Ambien and 30 10 mg of Lexapro with 1 refill. Karmen is a 50-year-old married who told her psychiatrist that she was considering suicide through overdosing on Advil. She complains of severe back pain that has left her with a “poor mood”. She talked about the injury for a long period of time. When doctors did not validate her injury, she described feeling abandoned. Karmen had gained weight and was upset about that. She did not take making suicidal comments seriously and often just used them as a threat towards her husband. She craved the attention of the doctors, and was flirtatious with the person who interviewed her. Karmen’s husband said that she talked about suicide on a regular basis. Karmen became sexually active early in life and has always gone for older men.
Introduction: Laura Copeland is a twenty four year old Peace Corp representative in Ethiopia. During her work, her health seemed to be “off” (Magic Foundation 2014). Some of the odd health effects were extreme weight gain, deep purple stretch marks, slow wound healing, hair loss, “severely depressed with suicidal ideation”, and a ceased menstrual cycle. Her manager and the other people involved with her care became severely concerned. She was flown out to Washington D.C. to get a medical evaluation, and was placed on suicide watch in the mental ward.

One case that I have worked with at LINK involves an 18-year-old female. When discussing the client’s background and family situation it can be slightly confusing. In the client’s case record, it states that the client was adopted by a family when she was six years old and came to the United States to live with her adoptive parents. The client record also states that she does not have contact with the adoptive parents and that she eventually became homeless and stayed with a mentor who she now calls mom. According to the client, there is no contact with her biological mother and her adoptive parents are currently in Israel, but may be coming to the United States. 12/7/2015: Josephine Sullivan made a request that I speak with the patient about her depression. The patient is a divorced 45 year old woman; she states that she was working at the Pentagon on 9/11 and that she was later diagnosis with PTSD and Bipolar. Ms. Glaraga was here at this appointment seeking psychotrophic medications which she had discontinued 3 – 4 months ago because she didn’t like taking the medications and felt that she was doing better. Within the last month the patient has been experiencing symptoms severe depression, she scored a 18 on the PHQ-9 after answering the questions in writing herself. The social workers in both videos gathered information regarding each of the client’s issues. Another common denominator in both videos is that both of the social workers repeated what the client had said in their own words to allow the client to feel heard and understood. Although it seems easy to define, social work is a widespread profession that reaches many more areas than most realize. It can range from working for individuals, to all of society. The use of theories in social work is a large part of understanding and carrying out practice. Systems theory is a widely known theory used in social work, but brings debate upon whether it should be the main theory used in social work practice or not.
On July 7th I had an interview opportunity with the Oasis Shelter in Owensboro, Kentucky. This was one of the numerous opportunities that I placed on my possible field interest to receive an internship at. I met with the director of the Oasis shelter in Henderson, Kentucky that afternoon for the interview. The interview went well, and I informed the director that I was a Sociology undergrad student looking to expand my knowledge with domestic violence.
