The content of the course has proven to be invaluable, insightful and thoughtful. Observing and interacting with my cohorts gave relevance to information that was being presented in a contextual manner of the topics that were being presented. I was given the opportunity to participate in discussion that garnered participation from different facets of a particular discussion.
Leaders bear the burden of ensuring that things change in a positive direction. But by its very nature, innovation and technology can be disruptive, technology increase the chances of moving away from what is, to what can be, to what will be. Disruption technology is also known as change and is certainly uncomfortable.

Introduction “Health informatics is the science that underlies the academic investigation and practical application of computing and communications technology to healthcare, health education and biomedical research” (UofV, 2012). This broad area of inquiry incorporates the design and optimization of information systems that support clinical practice, public health and research; understanding and optimizing the way in which biomedical data and information systems are used for decision-making; and using communications and computing technology to better educate healthcare providers, researchers and consumers. Many new technologies are being used in health organizations across the nations, which are being utilized to help improve the quality of health care. Electronic Health Records (EHRs) play a critical role in improving access, quality and efficiency of healthcare ("Electronic health records," 2014). In order to assist in expanding the use of EHR’s, in 2011 The Centers for Medicaid and Medicare Services (CMS), instituted a EHR incentive program called the Meaningful use Program. This program was instituted to encourage and expand the use of the HER, by providing health professional and health organizations yearly incentive payments when they demonstrate meaningful use of the EHR ("Medicare and medicaid," 2014). The Meaningful use program will be explored including its’ implications for nurses, nursing, national policy, how the population health data relates to Meaningful use data collection in various stages and finally recommendations for beneficial improvement for patient outcomes and population health and more.
The world of healthcare changes every day. Technology, as we know it evolves and changes the actual care that patients receive and even post care as well. It has been determined that most faults are caused by system failures. When a break in the system has occurred it must then be decided where the,” inefficiencies, ineffective care and preventable errors” to then influence changes within the broken system (Hughes, 2008). Improvements sometimes can begin with measurements and benchmarks which in turn will allow organizations to assess the trouble spots and broken areas within the system. “An electronic health record (EHR) is a digital version of a patient’s paper chart. EHRs are real-time, patient-centered records that make information available instantly and securely to authorized users.” (healthit.gov) The EHR mandate was created “to share information with other health care providers and organizations – such as laboratories, specialists, medical imaging facilities, pharmacies, emergency facilities, and school and workplace clinics – so they contain information from all clinicians involved in a patient’s care.” ("Providers & Professionals | HealthIT.gov", n.d., p. 1) The process has proved to be quite challenging for providers. As an incentive, the government began issuing payments to those providers who “meaningfully use certified electronic health record (EHR) technology.” (hhs.gov) There are three stages that providers must progress through in order to receive theses financial incentives. Stage one is the initial stage and is met with the creation and implementation of the HER in the business. Stage two “increases health information exchange between providers.” ("United States Department of Health and Human Services | HHS.gov", n.d., p. 1) Stage three will be the continuation and expansion of the “meaningful use objectives.” ("United States Department of Health and Human Services | HHS.gov", n.d., p. 1) The hospital, where I work, initiated the HER mandate many years ago. In this paper, I will discuss the progression and the challenges that my hospital encountered while implementing the EHR mandate.
Health information technology must be used as the powerful tool it is to attain the ultimate goal of generating improved healthcare quality and public health outcomes. The NQS outlines objectives and concerns for improving health equality and health care quality the same way. It implies that the quality improvement being enabled through health information technology will support the goals envisioned by the NQS (Taylor, 2015).
Data collection should be carefully managed in healthcare organizations. Time spent collecting data can consume huge portions of a provider's day -- taking him or her away from more direct patient care activities. Other employees may spend their entire day collecting data. When you consider the cost of data collection equipment, software, employee time, benefits, and other overhead, the price of data collection can add up quickly. And what are you getting for your money? Is the data collected reliable? Is it comprehensive? Does it provide the necessary detail to answer important clinical and business decisions? For the price your facility is paying, the answers to these questions must be yes.

This paper will identify the use of Electronic Health Records and how nursing plays an important role. Emerging in the early 2000’s, utilizing Electronic Health Records have quickly become a part of normal practice. An EHR could help prevent dangerous medical mistakes, decrease in medical costs, and an overall improvement in medical care. Patients are often taking multiple medications, forget to mention important procedures/diagnoses to providers, and at times fail to follow up with providers. Maintaining an EHR could help tack data, identify patients who are due for preventative screenings and visits, monitor VS, & improve overall quality of care in a practice. Nurse informaticists play an important role in the adaptation, utilization, and functionality of an EHR. The impact the EHR could have on a general population is invaluable; therefore, it needs special attention from a trained professional.
My overall vision is to develop and promote information technology solutions to better improve health outcomes, patient safety, and prevention of medical errors in underserved countries. By the late 1990s, computer-based patient records (CPR) were replaced with the term EHR (Wager et al., 2009).
A rising concern with informatics and public health is the barrier between data sharing. A major challenge for public health informatics is facilitating the improved exchange of information between public health and clinical care. Many of the data in public health information systems still come from forms filled out by hand, which are later computer-coded. Some reports are electronic but the initial data still have to be entered manually, this results in serious underreporting of data. Information silos typically do not share priorities, goals or even the same tools. Departments operate as individual units; silos occur due to an organization structure. Silos make it difficult to share information, agencies store same information in multiple places. Furthermore, silos increase health agency cost.

Informatics is known as putting data into information (Hebda & Czar, 2013). This is significant to nursing and health care because healthcare workers use informatics everyday. Through the use of technology, data is taken and transcribed into information in healthcare. I will use software to input a client’s information; I can use it to communicate with other healthcare workers and share needed information. Informatics develops my career as a nurse and helps me advance the maximum effort I can put in as a healthcare worker. Whether its using an electronic blood pressure machine or reading x-rays from a computer, informatics plays a significant role in healthcare. There was a significant amount of knowledge given in this course, involving concepts that in our teaching career will be extremely important. From this course I have gained a significant amount of learning experiences.
