Monitoring a health and social care team’s performance is essential so that any threats to its service delivery, are identified early and corrective actions taken. There are two dimensions of team functioning: the tasks the team are required to do, and the social climate that impacts on how they operate. The social climate will determine how team members cope with diversity of opinions, accept difference and resolve conflict. Monitoring and evaluation are integral parts of management and provide a link between planning and implementation. While monitoring focuses on the activities and outputs, evaluation focuses on the outcome and goals. Monitoring focuses on inputs and outputs and is the continuous process of gathering information to measure against the goals and objectives. Evaluation, like monitoring, is a continuous process. Evaluation provides feedback on whether plans have been met and the reasons for success or failure. Evaluation should highlight shortcomings so they can be identified and corrected. It should provide direction for future plans. It is important to monitor and assess the on-going performance of teams and individuals, in order to evaluate the progress towards agreed objectives. D1: I have decided to look at a 6 year old going through bereavement. Bereavement means to lose an individual very close to you. When children go through bereavement they are most likely to feel sad and upset about the person’s death. Children at a young age may not understand when a family member dies. Children may not understand bereavement. For example a 6 year old’s father been in a car crash and has died from that incident. Death is unpredictable and children can’t be prepared for a death of a family member as no one knows when someone is going to die or not. Unfortunately every child can experience bereavement even when a pet dies. It is important that we are aware that effects on the child so we can support them in the aftermath.

The theory used for this case study was effective, because it was able to get P to recognise that change need to occur so the support that she received would be effective. Reflecting on action the outcome of the assessment and intervention that was applied to P’s case I used supervision and to discuss concerns I had regarding this family with my PS. Supervision helped me establish my concerns and areas I required further support in application to theory in evidence based practice ‘blind self’.

This piece of work will be based on the pre-assessment process that patients go through on arrival to an endoscopy unit in which I was placed in during my second year studying Adult diploma Nursing. I will explore one patient’s holistic needs, identifying the priorities of care that the patient requires; I will then highlight a particular priority and give a rational behind this. During an admission I completed under the supervision of my mentor I was pre-assessing a 37 year old lady who had arrived to the unit for an upper gastrointestinal endoscopy. During the pre-assessment it was important that a holistic assessment is performed as every patient is an individual with unique care needs as the patient outline in this piece of work has learning disabilities it was imperative to identify any barriers with communication (Nursing standards 2006).

For the Health Assessment and Health Promotion portion of NURS 255, we were paired with 66 year old G.T., a resident of the senior living community Freedom Village. Over the course of two visits, we were able conduct a health history and physical assessment on our client, who was in outstanding health in comparison to her other individuals her age.
