The goal of case management is to come to the point that the client’s to the point of termination of case management and the client is released. There are different strategies the case management can use in order to ensure his or her client remains successful after termination has been made. Strategies that can be used is communicating with everyone involved in the case including family and friends, risk assessment, and setting up community resources before termination has been made. It is also important to encourage and set up independent care in order to prompt client’s growth. If he or she does have major risk concerns this needs to be addressed upon release in the effort to reduce these risk. This will help to also ensure the proper community resources are being used or recommended for your client. During the assessment all community recourses need to be contacted to ensure a proper assessment/ evaluation is completed including talking will all the people that work with your client. Once a proper risk assessment/ evaluation has been accomplish the case manger can move forward with termination of case management depending on the assessment or court order of when termination of case management is. You can also set up more or less community resources the client can use once case management has been termination once the assessment is done. Like for the assessment he or she needs to contact all community resources and follow up with his or her client. Also the case manger needs to contact community resources and ensure everything is set up for the client before termination of the case management. Getting everything set up for the client can make the transaction easier for the client. This will also help the client to feel more comfortable and to know that even know case management was being terminated they are not going to be just thrown out into the community without being able to reach out and get help at anytime (Biel, 2013).
Before case management termination takes place it is important to start releasing the client gradually from services he or she may not need or need as much of. As a case manager we are “to coordinate needed services provided by any number of agencies, organizations, or facilities” (Kirst-Ashman & Hull, 2015, p. 31). Not only did she advocate for health services for Brenna but she also working on her housing issue, helped her set up a monthly food budget, helped her find counseling, and helped her build a support network (Kirst-Ashman & Hull, 2015, p. 32). My understanding of case management comes from an accumulation of lecture, readings, and a little bit of research. At first I thought case management meant to manage a case, which it kind of does, but it is a lot of background work that goes unnoticed from the workers part. One thing for sure I can say about case management is that is a very stressful and demanding job for the worker, therefore, you have to be a responsible worker, so that your client can hopefully get the services and resources he or she may need.
