Electroconvulsive therapy (ECT) was first developed in 1938. It has a history of abuse, exploited as a means of punishing or controlling people in mental hospitals, consequently ECT had poor reputation with negative depictions, but since then it has drastically improved with confirmed effectiveness. Despite the improvement in techniques, the use of ECT continues to decline since the National Institute for Clinical Excellence (NICE,2003) states that it should only be administered to severely depressed patients for whom psychotherapy and medication have proved ineffective or when the condition is considered potentially life threatening. For example, when there is a risk of suicide or risk to others because ECT has much quicker results than any other forms of treatment for depression. Overall, the aim of this PowerPoint is to provide information on how ECT is administered, how or why it works, benefits, side effects, appropriateness and effectiveness.
Before ECT begins, patients are given a short-acting anaesthetic and a nerve-blocking agent, paralysing the muscles of the body to prevent them from contracting during the treatment and causing fractures. Oxygen is also given to patients to compensate for their inability to breathe. Then they receive either unilateral ECT or bilateral ECT. In unilateral ECT, an electrode is placed above the temple of the non-dominant side of the brain, and a second in the middle of the forehead. Alternatively in bilateral ECT, one electrode is placed above each temple. After this, a small amount of electrical current (approximately 0.6 amps) is passed through the brain, lasting about half a second. In summary, despite controversies over the administration of ECT for the treatment of depression, the techniques have been modified and improved, conversely making ECT a fairly safe and effective treatment for severe depression, providing a quick relief from suffering caused by severe depression. Although there are controversies regarding how ECT works, this PowerPoint presentation provided two alternative theories to provide explanation of the mechanisms underlying actions of ECT: the neurotransmitter theory and the hyperconnectivity theory, both of which are supported by research evidence. However there are side effects associated with ECT that need to be addressed and also it fails to provide a long-term cure thus it needs to be administered in conjunction with psychological treatments of depression.

An electrical current is passed through the brain. This is thought to change the brain chemistry and increase the mood. People with Bipolar Disorder are encouraged to avoid drinking alcohol, avoid the use of street drugs or misusing prescription medications, avoid unhealthy relationships, get plenty of sleep, and exercise on a regular basis. One thing is clear. The person themselves must be active in their own well-being in order to maintain a relatively healthy and productive lifestyle. According to Sharf, (2008) the eye movement desensitization and reprocessing (EMDR) was designed to treat posttraumatic stress disorder. EMDR requires that the clients visualize an upsetting memory and accompanying physical sensations. The clients repeat negative self-statements that they associate with the scene. The procedure is repeated again and again until the client’s anxiety is reduced. EMDR focuses on desensitizing strong emotional reasons in clients and help them to reframe their belief systems to accommodate new emotional states (Sharf, 2008).

Currently, there is a lack of studies in regards to whether or not this therapy causes significant damage to the brain. Correspondingly, there is little research done in regards to how great relapse rates are and how long the treatment can truly last. Consequently, many people have abstained from receiving treatment to avoid any possibility of impairment or reversion. However, it has not been denied nor confirmed that ECT directly causes the aforementioned results. Moreover, many theories created to downplay electroconvulsive therapy’s effectiveness are being brought back into the spotlight, including one that attributes ECT’s success to brain damage. Breggin states, “More recently [Harold] Sackeim and Sackeim with a team of colleagues have covertly revived the principle that a therapeutic response depends upon the degree of brain damage and dysfunction” (par. 17). Although this theory was made to discredit ECT, there has been no research done to disprove its accuracy. Furthermore, because of a lack of research, when a new study such as this comes out, many people believe it right away no matter how erroneous it may actually be. Diagnosing Epilepsy can be a long process with lots of steps to follow. When first going thru the process there will need to be a confirmation of parent history. Then there will be a full neurological exam followed by blood and clinical tests, to make sure it wasn’t some other type of episode such as fainting. Apart from the neurological exam, the EEG is the best tool to diagnosing seizures and epilepsy. Then the doctor will identify the type of seizures and do a clinical evaluation to determine the cause of epilepsy. Now depending on the results they will determine the best type of treatment.
Depression is a mental illness, which affects millions of Americans each year. Currently there are many prescription drugs, called anti-depressants that have been proven to successfully treat it. The causes of depression are somewhat of a medical enigma, however, it is known that depression is associated with a change in the brains chemistry involving the function of neurotransmitters (Reichert). This chemical change occurs in healthy brain’s, which experience sadness, but ends after the unpleasant stimulus is removed. In people suffering from depression this chemical change does not correspond to any particular stimulus. Symptoms of depression are often incapacitating and include severe and extended sadness, feelings of worthlessness, feelings of emptiness, irritability and anxiety (Reichert, Spake).
Several strides have been made in the medical field in regards to electroconvulsive therapy. The treatment is defined as “a medical procedure in which a brief electrical stimulus is used to induce a cerebral seizure under controlled conditions” (Enns, Reiss & Chan, 2010). It is used for a handful of mental illnesses, such as major depressive disorder, bipolar disorder, and schizophrenia, but when it was introduced in 1938 by Ugo Cerletti and Lucio Bin, its main use was to treat schizophrenia (Enns, Reiss & Chan, 2010). ECT can indeed be effective for schizophrenia, but the best results are when the “… duration of [the] illness is relatively brief or when catatonic or affective symptoms are prominent” (Enns, Reiss & Chan, 2010). ECT is a biomedical therapy that sends an electric current through the brain of an anesthetized patient. Another controversial method, rTMS, uses magnetic energy pulses to stimulate or suppress brain functioning and activity. However, through the process of communication, chemical messengers, also known as neurotransmitters can suddenly fail, resulting in what is known as a seizure attack. Epilepsy occurs when a few too many brain cells become excited, or activated simultaneously, so that the brain cannot function properly and to it’s highest potential. Epilepsy is characterized when there is an abnormal imbalance in the chemical activity of the brain, leading to a disruption in the electrical activity of the brain. This disruption specifically occurs in the central nervous system (CNS), which is the part of the nervous system that contains the brain and spinal cord. This causes an interruption in communication between presynaptic neurons and postsynaptic neurons; between the axon of one neuron, the message sender and the dendrite of another neuron, the message recipient. The diagnosis of epilepsy is usually made after the patient experiences a second unprovoked seizure (Leppik, 2002). Diagnosis is often difficult, however, since it is unlikely that the physician will actually see the patient experience and epileptic seizure, and therefore must rely heavily on patient’s history. An electroencephalography (EEG) is often used to examine the patient’s brain waves, and some forms of epilepsy can be revealed by a characteristic disturbance in electrical frequency (Bassick, 1993). The variations in frequency can take form as spikes or sharp waves (Fisher, 1995). The variations are divided into two groups, ictal electrograph abnormalities, which are disturbances resulting from seizure activity, and interictal electrograph abnormalities, or disturbances between seizures. Electrical stimulation (ES) therapy involves the transfer of electrical current across wound tissue through the two electrodes and uses three types of currents: AC, DC and Pulse DC. According to various studies, electrical stimulation therapy has shown to reduce inflammation, edema, improve wound appearance, cause a faster reduction in wound size by helping to kick start the wound healing process and is cost effective for many patients. It has generated a positive impact on patient’s comfort and convenience. Electrical stimulation therapy has provided numerous beneficial effects in wound healing demonstrated by various studies, however; further research trials are needed in order to prove the effectiveness and benefits of electrical stimulation therapy as it relates to wound healing (Cutting, 2006).
For those who are receiving ECT, how exactly does it work? There are a few theories as to how ECT is effective for severe disorders such as depression. One theory would be the neurochemical theory of the mechanism of action of ECT. This theory is based on the observation of large amounts of hypothalamic and pituitary hormones released within minutes which is then measurable in cerebrospinal fluid and blood, however, there tends to be a certain criticism that is often cited against this theory (Gomez, 2004, p. 476). That criticism would be the lack of identification of specific behavior regulating substances, similar to insulin (Gomez, 2004, p. 476). As I said, though, there are a few theories so let’s take a look at another.
