The Hmong people, an Asian ethnic group from the mountainous regions of China, Vietnam and Laos, greatly value their culture and traditions. The film “The Split Horn: Life of a Hmong Shaman in America” documents the seventeen year journey of the Hmong Shaman, Paja Thao and his family from the mountains of Laos to the heartland of America. This film shows the struggle of Paja Thao to maintain their 5000 year-old shamanic traditions as his children embrace the American culture. Moreover, the film shows that one of the major problems refugees like Paja Thao and his family face upon their arrival to the United States is conflict with the American medical system. The Hmong culture is evidence that health worlds exist. Health worlds exist in which health is understood in terms of its social and religious context (SITE BOOK). Spiritual beliefs in the Hmong culture are strongly connected to their view and description of health and illness (SITE 6). Illness in the Hmong culture is believed to be caused by evil spirits, a curse from an unhappy ancestor, or a separation of the soul from the body (California Department of Health Services, 2004). Paja Thao, the shaman in “The Split Horn” emphasizes his belief that a soul can separate from its body and the failure to return back to the body is a sign that the individual will become ill. Like the Chinese concept of ‘Ying and Yang’, Hmong people believe that the balance between the body and soul determines perfect health. Paja Thao believes that a body is attached to seven souls, and when there is a loss in a soul, illness occurs. In contrast to this holistic concept that the Hmong’s believe in, the Western culture is not able to predict when illnesses will occur. The dominant biomedical model of health does not take into consideration lay perspectives (SITE BOOK). Lay perspectives go into detail about ordinary people’s common sense and personal experiences. A cultural perspective, like the Hmong cultures perspective on health, is considered a lay perspective. Unlike the Hmong culture, where illness is viewed as the imbalance between the soul and the body, the dominant biomedical model of health views health in terms of pathology and disease (SITE THE BOOK). Although the Hmong culture considers spiritual and environmental factors, the dominant biomedical model of health only looks at health through a biological perspective, and neglects the environment and psychological factors that affect health. Depression in the U.S. is a medical illness caused by neurochemical or hormonal imbalance and certain styles of thinking. The cultures between the Swedish and the Hmong people follow a similar path of struggle, conforming, and success. The Swedish people struggled from famine and needed a place to go to where jobs were abundant. While the Hmong also struggled from war and the after effects of war. These struggles brought a group of people with different cultures to one country, America. America became a place of refuge and a place to prosper. Many of the people succeeded in getting high paying jobs and would help America evolve into a better place. Although, this succession gave people a new hope, there are still discrimination within the country because of differences of culture and looks.

The Hmong, an Asian ethnic group, came from an oral culture, where they did not have any written form until the 1950s (McCall, 1999). The Hmong lived an agricultural lifestyle in the hill and mountain areas in Laos, Vietnam, and Thailand (McCall, 1999; Tatman, 2004). They focused on physical labor to provide food for the family and lacked formal education, as it was not essential (Lee & Green, 2008; McCall, 1999).

Depression is an illness which not only effects a person emotionally, it also effects a person physically. Different cultures view depression different ways. The Hmong culture is firmly rooted in their spiritual belief of animism, ancestral worship and reincarnation. These beliefs connect them to their sense of health and well-being. They view illness as having either a natural or a spiritual cause. Foua and Nao Kao believed that the reason a Hmong person would fall ill, was because a “dab” had stole their soul. Doctors in Merceds didn’t understand this concept and that is why Lee’s felt many Hmong patients didn’t have good health outcomes. I got the impression that many of the individuals involved in the care of Lia and her family, put up a wall before they even entered the situation. There was a blatant lack of communication and understanding about the Hmong values and beliefs, which lead to ineffective care for Lia. Both the Lees and the American doctors were doing what they believed was best. Unfortunately, the cultural barriers that were created between these two groups didn’t allow for many positive outcomes. In the past few decades, clinical treatment has moved on from biomedical approach to the biopsychosocial approach. Scientists now believe that health of an individual is not merely due to physiological conditions but may also depend on psychological and socio-cultural perspectives(Engel, 1977). The term health psychology was developed by the American Psychological Association to describe health and wellness issues that arise from psychological problems. Health psychology stems from Systems theory, which says that human events such as injury and illness exist within several interconnected systems, such as our peer relationships, childhood experiences, personality and other factors (Schwartz, 1982).
Mental illness is an addition to all of the previously listed perceived disadvantages of Native Americans by those of other ethnicities. Many believe that Native Americans are at a higher risk for mental illness than those of European descent. Many also believe that Native Americans have more people suffer from depression than their white counterparts (Stark & Wilkins, American Indian Politics and the American Political System, 2011). There have been studies conducted to test whether or not this is the case, with mixed results. Some studies say that Natives are at a higher risk and others say they are not. This discrepancy makes the answer unclear. If Natives are actually at a higher risk for and have more people suffering from depression than individuals of European descent, the question to ask is, “why?” Several factors play into depression and other mental illness, including biology, social standing, history, family, and any preexisting/comorbid diseases that could contribute to or cause depression.
Anne Fadiman’s book The Spirit Catches You and You Fall Down is the embodiment of a clash between two cultures that are worlds apart. Cooperating effectively and understanding individuals with different backgrounds and traditions enhances the quality of health care provided by hospitals and other medical facilities. One of the many cultures that nurses and other health care providers encounter is the American Indian or Native American culture. There are hundreds of different American Indian Tribes, but their beliefs and values only differ slightly. The culture itself embodies nature. To American Indians, “The Earth is considered to be a living organism- the body of a higher individual, with a will and desire to be well.
