Underweight ix. Absence of appropriate immunizations for age x. Genu varum xi. Borderline lead toxicity 2. For each problem identified discuss plans including referrals, test, pharmacologic and non-pharmacologic management. a. Social i. Speech- At this age patient should be able to form three to four word sentences, and words should be 90% intelligible by a stranger (Murphy & Berry, 2013). Before referring the patient for therapy, provider needs to use a diagnostic test tools to help further determine the degree of the delay. The Denver Articulations Screening Exam could be used to screen the patient for articulation problems, along with the Clinical Evaluation of Language Fundamentals-Preschool which screens for both receptive and expressive language problems. Furthermore, the custodial caregivers need to be encouraged to stimulate the child. This can be done by reading to the child daily, displaying appropriate language, using clear simple sentences and language, and providing the child opportunities to interact with other children. Likewise, television viewing needs to be limited, and caregivers need to continually provide encouragement. Teach that with age most language errors disappear.
