Policies and procedures in nursing practice are important for nurses to perform and to deliver quality of care and patient safety. Policies and procedures help nurses to prevent errors and carry out procedures safely to the patients. It also brings uniformity in following organizational guidelines. Policies and procedures are designed to influence and determine major decisions and actions, and activities that take place within the boundaries set by them. Procedures are the specific methods employed to express policies in action in day to day operations. Policies and procedures are made for the users easily understandable, well organized, and easily accessible (Policies and procedures - Definition, 2012). Nowadays, most hospitals and clinics maintain their nursing policies and procedure manuals on campus intranet/public drives and also on the units. This writer as a nurse leader has access to use these resources. Evidence based policy has been defined as an approach that “helps people make well informed decisions about policies, programs and projects by putting the best available evidence from research at the heart of policy development and implementation”(Davies, 1999). This nurse leader’s facility utilizes policies that evidence based and retrieved from best available resources.
One day, this writer happened to see another nurse changing a Peripherally Inserted Central Catheter Line dressing. As a nurse leader, this writer asked the nurse why she is changing the dressing. The NMBA sets out Statements of Principles which provide guidance to nurses regarding processes that will help to ensure that ‘safety is not compromised’ regarding decision making about nursing practice. According to the NMBA, the fundamental motivation for any decision about a care activity is to meet clients’ health needs or to enhance health outcomes. Decisions regarding activities are made in a planned and careful manner and: ‘only where there is a justifiable, evidence-based reason to perform the activity’ (NMBA: 2012, p.6). Furthermore, the NMBA points out that nursing practice decisions are more effective in a collaborative context of planning, risk management, and evaluation. Thus, organisational employers/managers, other health workers and nurses’ work together in sharing a combined responsibility to design and maintain: environments (including resources, education, policy, evaluation and competence assessment) that support safe decisions and competent, evidence-based practice to the full extent of the scope of nursing practice.
According to ASHA Evidence Based Practice is the combination of clinical expertise opinion, data, and patient’s perspectives, with the goal of providing high-quality services (2013). The process of evidence-based practice consists of formulating a research question, collecting evidence, including views, and then evaluating the entire process. This project introduces the research topic I’ve chosen, which identifies a researchable problem, and formulates an answerable question that is relevant to nursing and evidence-based practice.
As an ICU nurse I constantly watch how patients develop pressure ulcers, a pressure ulcer is an area of skin that breaks down due to having constant friction and pressure, also from having limited movement and being in the same position over a prolonged period of time. Pressure Ulcers commonly occur in the buttocks, elbows, knees, back, shoulders, hips, heels, back of head, ankles and any other area with bony prominences. According to Cox, J. (2011) “Pressure ulcers are one of the most underrated conditions in critically ill patients. Despite the introduction of clinical practice guidelines and advances in medical technology, the prevalence of pressure ulcers in hospitalized patients continues to escalate” (p. 364). Patients with critical conditions have many factors that affect their mobility and therefore predispose them to developing pressure ulcers. This issue is significant to the nursing practice because nurses are the main care givers of these patients and are the ones responsible for the prevention of pressure ulcers in patients. Nurses should be aware of the tools and resources available and know the different techniques in providing care for the prevention of such. The purpose of this paper is to identify possible research questions that relate to the development of pressure ulcers in ICU patients and in the end generate a research question using the PICO model. “The PICO framework and its variations were developed to answer health related questions” (Davies, K., 2011).
The goals and objectives set out in the policy and subsequently analysing one aspect of the policy in particular are included. How patient care and nurses delivery of said care is improved by this policy.
Central lines (CL) are used frequently in hospitals throughout the world. They are placed by trained health care providers, many times nurses, using sterile technique but nosocomial central line catheter associated blood stream infections (CLABSI) have been a dangerous issue. This is a problem that nurses need to pay particular attention to, and is a quality assurance issue, because CLABSI’s “are associated with increased morbidity, mortality, and health care costs” (The Joint Commission, 2012). There have been numerous studies conducted, with the objective to determine steps to take to decrease CLABSI infection rate, and research continues to be ongoing today. The problem is prevalent on many nursing units, with some patients at great risk than others, but some studies have shown if health care providers follow the current literature, or evidence based guidelines, CLABSIs can be prevented (The Joint Commission, 2012). The purpose of this paper is to summarize current findings related to this topic, and establish a quality assurance (QA) change plan nurses can implement for CL placement and maintenance, leading to decreased risk of nosocomial CLABSIs.
The healthcare system is very complex, and the nurses should be aware of all the policies, laws, ethics, and available sources to provide quality care to all patients. The following case study will explore some of the decision-making processes the nurses consider while caring for their patients to keep high standards of care.

As nurses we need to continually expand our skill levels, gain new knowledge and expand our technological and scientific practice potentials. Nurses need to be willing to commit time to research, seeking out evidence based practices and become change agents within their communities, organizations, local, state and federal governments. I believe it is a nurse’s responsibility to inform and educate patients. In my opinion, this is an example of translating research into practice, which I believe to be an important nursing role, in healthcare

Evidence based practice (EBP) is a key component in delivering cost-effective, high quality health care. [1] However, only around half of the care providers in the United States utilized EBPs. Additionally, nearly a quarter of services delivered to American consumers are unnecessary and potentially harmful. Today, educators are teaching and promoting evidenced based health care to future nursing professionals. The Institute of Medicine (IOM) supports this action as a means to achieve the objective that 90 percent of all medical treatments have a foundation in evidence based practices by the year 2020.
1.The characteristics that intrigued me of the nursing practice was as a patient at the Danbury Medical Center. As a patient, the nurses gave me the most delightful patient care ever received. They took their time to make sure my care was an urgency. When my pain had increased the nurses were there by my side. They had taken the time to make sure the proper dose of medicine was given to me at the correct time. Even though my current status was lying in bed with a fractured femur. They took the time to make sure my needs were met. I had never expected that going to the hospital from flying off a cliff on my skateboard would direct me in my future dream job. The nurses showed me what patient care really and truly was. Patient care is putting others in front of your own needs. Being a great nurse is showing your patient that there is hope. Patient care is not only making sure your patient is satisfied but making sure their family and loved ones are cared for as well.
The nursing profession has changed drastically over time. The roles and responsibilities that nurses take on have increased and become far more complicated. Nurses are managers, leaders, supervisors and have become experts in many areas of care. Every day nurses are faced with the task of improving and strengthening professional leadership within their work environment. Managing good quality and eliminating risk is the major challenge in health care. All members of the team must work together to accomplish outstanding patient care. Budget cuts and nursing shortage in all areas of health care leads to less licensed staff, where use of unlicensed personnel have been used widely, where delegation is not an option, but a necessity. Nurses must be aware of delegation guidelines, what tasks to delegate,when to delegate for the safety of patients, liability of nurses and the facility.
