When we think about patient safety, we thinking about how hospitals and other health organizations protect patients from accidents and injuries. Patient safety is a very serious public health issue. Studies show that out of every 10 patients, 1 can/will be harmed while receiving care. The most common safety issues are infections, falls, medication errors, wrong site surgeries, and readmissions (“What is Patient Safety?”). Falls are in the top 10 patient safety issues along with infections, surgery issues, medication errors and misdiagnosis. Falls are recognized as one of the most common geriatric issues that causes problems and jeopardizes the self-reliability of elders. 30-40% of individuals over 65 years of age fall yearly, this percentage is higher for nursing home occupants. Falls can be related to increased morbidity, mortality, and nursing home placement. Over 1800 nursing home occupants die from falls yearly. Even though only 5% of individuals over 65 live in nursing homes, this small number accounts for 20% of fall-related deaths for this age group (“Strategies for Reducing Falls”).
This issue deserves attention based on the number of falls a year alone and because falls often end in disability, health decline, decreased quality of life, and sometimes death. Fear of falling can also cause increased loss of function, depression, and altered mental status, therefore finding a way to prevent and decrease nursing homes falls is very important. Prevention of falls in nursing homes is a major concern in regards to quality improvement in patient safety. Research shows that elders fall at least 3 times a year (Willy and Osterberg, 2014).

Education, training and mandatory reporting of falls can help with this. Fall prevention is important but also very challenging to reduce and eliminate. Reducing the risk of falls in nursing care facilities requires identification of risk factors; and this starts at admission. Knowing the root cause of the fall, keeping up with when, who, where and how the fall happened in the first place and documentation. Other ways to assist in reducing the risks falls include:
• Documenting near known faller’s room
• Increasing staff
• Allowing staff extra time related to shift changes
Staff should have proper training on what to do when and if a resident falls. Staff should know the correct safety techniques, this can prevent further injury (“Preventing falls”).
Insights gained
There are many strategies that nursing home staff can use to prevent falls and increase safety. For example, Illinois Council homes uses the following guidelines in their facility to help reduce falls:
 Recognize high risk residents, residents that have poor vision, bone issues, heart disease, and a history of falls.
