Introduction
The health care system and nursing practices are undergoing rapid changes, which are as well becoming complex. Preparing nurses balance these changes is a significant challenge. The Institute of Medicine (IOM) report explored how nursing profession has to change to bring out a more solid health care system. As a result of these changes, the Institute of Medicine (IOM) decided to examine “Future of Nursing, Leading change and Advancing Health” (Creasia & Fribery, 2011). This paper will discuss the impact of the 2010 IOM report on nursing Education, nursing Practice with emphasis on primary care, its effects on present nursing practice, and the nurse’s role as a leader.
Effect of IOM report on nursing education
The IOM reported the aim of future of nursing is to create improvement in the health of Americans through nursing profession, also to meet need of the different environmental health settings in which they will practice. It is important nurses get advanced education and training. IOM recommended “80 per cent of nurses to get BSN in nursing by the year 2020” (Institute of medicine, 2010). Nurses are active, they see policies as what they can restructure or change, instead of taking it as what has been imposed on them. They are involved in policy development as well as working together as one profession. As nurses, they participating in decision making regarding health policies, as well as taking part in implementations associated with changes in health care. Currently, nurses are serving “on advisory committees, commissions, and boards” (Cohen, M. & Walker, A. 2010). Policies and decisions in these areas will help to advance patient care in health
The second key message focused on the recommendation that registered nurses receive greater levels of training and education. This message emphasized the need for nurses to enter the work force with a baccalaureate degree, or obtain it shortly afterwards. It also stressed the need more graduate nurses, specifically “nurse faculty and researchers at the doctoral level” (IOM, 2011, p. 164). Innovative solutions were devised to speak to nursing school capacity and curriculum in order to attain the IOM recommended goal of 80 percent of nurses holding a BSN by the year 2020.
The IOM report had four key messages needed for advancing the future of nursing. “Nurses should practice to the full extent of their education and training; achieve higher levels of education and training through an improved education system that promotes seamless academic progression; be full partners, with physicians and other health care professionals, …and; effective workforce planning and policy making require better data collection and an improved information infrastructure” (Institute of Medicine, 2011). The report also included eight recommendations needed to facilitate the necessary changes to in the nursing profession so meet to demands of the healthcare reform.

This week I have learned about the importance and nurse’s role in the healthcare policy making. Nurses provide essential services and knowledgeable about client needs. They interact with healthcare consumers in a wide variety of settings. This provide nurses a wide knowledge about patients health needs, an understanding of factors that affect the health of patient and their families and have insight into how people respond to various strategies and services. Nurses play a vital role in interpreting people's needs and expectations for healthcare. They also understand the expectations and needs of society's vulnerable groups. The 2010 Institute of Medicine (IOM) Report- The Future of Nursing described the role that nurses have in the current and future US health care environment (IOM, 2011). This report was completed at a time when the Affordable Care Act had been passed and a new emphasis was being put on interdisciplinary healthcare teams, care coordination, value-based payment systems, and preventative care (IOM, 2011). Nursing is the largest profession in health care and with an aging baby boomer populace, the expanded role of nurses will be critical in meeting the growing healthcare burdens (Sisko et al., 2014).
There has been a drastic cut in both the Medicare and Medicaid reimbursement rates. According to Robert (2012), “cuts of more than $360 billion to Medicare and Medicaid will be made over the next 10 years. The focus of nursing needs to emphasize more on wellness care and prevention rather than acute care” (McNeal, G., 2012). Nursing practice will need to shift more towards community and population focused nursing. Baccalaureate prepared nurses are provided with a curriculum that includes both community health and leadership skills that are not included in the associate program. By encouraging associate’s degree nurses to obtain a bachelor’s degree in nursing, an increase in the awareness of the needs of the community and population may be seen. The IOM report has outlined the anticipated obstacles that healthcare will face if changes are not made. By allowing nurses to provide care within their full scope of practice, quality care may be provided at an affordable cost to the population. The use of advanced practice nurses in primary care may provide quality, access, and cost efficient healthcare to high-risk populations and possibly decrease hospital admission rates, thus lowering the overall cost of healthcare. As young American adults turn eighteen, they are reminded that it is their civic duty and responsibility to register to vote, to be aware of current policies and to make educated decisions in the voting booth. As some of these young adults become active in the nursing profession, they are taught that their ultimate responsibility is to advocate for their patients; they are to ensure their safety and well-being. That being said, does this responsibility stop at the bedside? Should nurses take an active role in the policy making that directly affects the healthcare of their patients? Conn and Armer (2012) stated that “nurses must be prepared to use multiple and varied strategies to influence public policy so that optimal health outcomes can be achieved” (p. 296). This statement alone implies that nursing’s responsibility extends beyond direct patient care.
Profession of Nursing is always regards as a complex, dynamic and noble. The nursing profession requires many things such as hard work, a vast knowledge, excellent communication skills, and a passion for the noble profession. All these qualities are needed to be an effective nurse. Today, nurses are living in a world of ever changing field of medicine .The present patient statistics poses, nurses are expected to take responsibility of accomplishing the requirements. The requirement for excellent nursing skills is added as well as well-formed skills in different aspect. Because of the high demand in excellent nursing care, the IOM have declared the recommendations for the future of nursing.
Policy analysis encompasses the progress of ascertaining and evaluating alternative policies or program that intended to relieve or lessen economic, physical or social problems in the population (Patton, 2011). In determining the necessary steps of health care policy assessment that assist to shape the nursing practice environment, El-Jardali, et al., (2014) reported that policy analysis application offers results that are essential for the stakeholders and policymakers that can utilized in reviewing some of the laws in nursing laws in order to grasp an operative alternative that is feasible. Encouraging nurses to maintain a watchful eye on the current on health care policies and trends that may affect practice presents a challenge to the profession. As the workload for the average nurse increases it becomes difficult to dedicate time to policies. However, this is a fault that many must overcome and put forth effort as not being current on trends and policies may result missing a change leading to poor outcomes and patient harm. Facilities may have committees or groups that focus keeping up with current trends then must rely on the group to disperse information to the rest of the staff. Several years ago in 2008 the RWJF and the IOM collaborated for two years to discern the future needs of the nursing profession. Most importantly, the objective was to outline the critical actions needed to ensure nursing was ready to seamlessly move towards the future. This was no easy task as nurses work in such diverse settings such as outpatient areas, acute care settings, the community, and long term settings to name a few. Couple this with the fact that nurses have a variety of educational avenues such as the associates, diploma, or bachelor’s degree open to them to achieve the status of registered nurse (Institute of Medicine, 2010). All of this considered, the committee did design four key messages regarding the future of nursing as key in the transformation of health care as evidenced in their "Future of Nursing" report.
Issues and trends in the nursing profession are constantly changing due to discoveries in technology and research, changes in reimbursement and legislation, and the changing needs of our population; in fact the only constant in healthcare is change. This course has provided me with a foundation and better understanding of the need for higher education to empower nurses to become leaders and help shape the future of the nursing profession. Nurses are the “backbone” of healthcare and this is becoming more evident with the changes we are currently seeing in the healthcare industry.
