The urology team has experienced series of practice related concerns (manual pressure pump VS continuous pressurized pump). Patient and staff have reported safety risk incidents relative to the use of manual pump. Mr Sales volunteered to lead a quality improvement initiative related to patient and employee surgical safety. He evaluated the difference between the use of manual pump and continuous pressurized pump by soliciting the assistance of scrub techs, service lead RN/ST, flow coordinator, and medical representatives to collect information to identify a probable root cause of safety risk problems – handling and usage indication in urological surgical cases. He aggregated the data; identified evidence based studies, and provided recommendations to resolve existing problems. Through the utilisation of the accompanying text, I aim to discuss and reflect upon one person’s care, to which I have had previous involvement. Using the perspectives set out by Chapelhow et al (2005), I aim to gain a broader outlook on what is expected of me as a Student Nurse. Chapelhow et al (2005) has identified six skills that they feel are required of every Health Care Practitioner. These skills are defined as ‘enablers’ as they are fundamental skills that provide the means for expert professional practice (Chapelhow et al 2005). Although there are six skills outlined, I will only be using two of them to analyse the care I provided. I have chosen to use communication and risk management as my two enablers.

This protocol integrates the use of a portable bladder ultrasound to verify urinary retention prior to catheterization. The term “safety comes first” or more simply put, “safety first,” is a message that patients not only want to hear, but also want to know is the focus of the professionals that are caring for them; in particular, when they are under anesthesia and have limited or no ability to speak up or lookout for themselves. The National Patient Safety Agency (NPSA) has implemented two initiatives; Rocognising and Responding Appropriately to Early Signs of Deterioration in Hospitalised Patients (NPSA, 2007) and How to Guide: Five Steps to Safer Surgery (NPSA, 2010). Understanding that human beings make up the healthcare professional workforce, it is evident that tools and checklist can and will only be as good as the how people utilize and follow them. Thus, these initiatives “have been developed with consideration of human factors” (Beaumont & Russell, 2012). I know firsthand, that if my healthcare team would have followed these standards, I would have avoided torture, fear, and long term side effects from a routine hysterectomy procedure.
The role of a Registered Nurse cannot be neglected in the provision of quality and safe care to patients and adopt procedures adequate for the condition of the patients because they work at the front line level; moreover, they have direct dealing with patients and integration of personal and professional skills is necessary. Therefore, there are certain attributes that are necessary to be present in a Registered Nurse for accurately performing various tasks. These include; Workload management, leadership qualities, interpersonal skills, control of practice, professional development, effective communication skills and organi zational loyalty (Daly & Carnwell 2003, pp. 158-167). These attributes hold significance in terms of obtaining positive outcome for not only the Registered Nurse but also the organization and the patient. Workload should be managed in such a way that the care process is not affected. Registered Nurse should have leadership qualities to help, motivate and inspire other nurses. Similarly, a Registered nurse should also enable and promote learning opportunities for other nurses. A Registered Nurse has responsibilities towards the subordinates, patients and most importantly to the organization. Effective communication skills can allow Registered nurses to establish a trusting relationship with patients identifying their problems and needs. Mentors are ideal for nursing students and new nurses in providing support and guidance. They are able to provide effective communication, and help ease the transition into the workplace (Shellenbarger & Robb 2016). Furthermore, nursing mentors can help student nurses understand the social aspects and the flow of the hospital which can be pressuring. Nurses must be knowledgeable about the demands of this profession and how to deal with the pressure. Being aware of high demands of healthcare can help new nurses adjust in their positions, and improve quality care for patients. Therefore, knowing what’s expected results in nurses less likely to leave their position and decrease turnover rates. Job satisfaction is essentially important. Therefore, I strive every day to be that professional, that health care provider that has a positive and long lasting impact on patient’s health and wellbeing. I always strive to be that professional that my patients and colleagues trust, respect and admire. As a registered nurse, I have accomplished that. I know it because I am asked to precept new nurses and students, because my colleagues come to me frequently to ask me questions when they are in doubt, because when there is a difficult or complicated patient, my supervisor trust me with his care, because at the end of a long busy shift with seven to eight patients under my care, my patients praise me and thank me for the great and unique care I provided them.
The field of nursing provides one the opportunity to make a difference in the lives of others. Nurses interact directly with patients at times of hardship, vulnerability, and loss. The nursing profession has been around for decades. Due to the contribution from historical leaders in nursing, the nurse’s role has progressed over time. Although the roles of nurses have evolved throughout the years, one thing has remained the same: the purpose in giving the best patient care.

Time out was done by the anesthesiologist, the circulating nurse, the surgeon, and the scrub tech all pausing before the surgery and verifying the patient’s name and date of birth, the procedure being done, the site and location on the body in which the procedure was being done, and documented the count of all the equipment the scrub nurse had before surgery to compare to after surgery. 5. The patient’s privacy was protected and respected throughout the whole surgical procedure. The staff was very professional and I felt I learned a lot from them during my OR experience. 6. A sponge count is when the scrub nurse counts the sponges that are unused before the surgery she relays this to the circulating nurse and it is documented. After the surgery the count is redone to make sure that there are no sponges left in the patient. 7. The circulating nurse documents the information and signs the chart in the operating room. From pre-op to the operating room the nurses in pre-op gave off report to the circulating nurse by SBAR. From the operating room to PACU the anesthesiologist went with the patient and handed off the patient’s condition and information to the nurse in there. 8. In today’s society, leadership is a common yet useful trait used in every aspect of life and how we use this trait depends on our role. What defines leadership is when someone has the capability to lead an organization or a group of people. There are many examples that display a great sense of leadership such being an educator in health, a parent to their child, or even a nurse. In the medical field, leadership is highly used among nurses, doctors, nurse managers, director of nursing, and even the vice president of patient care services. Among the many positions in the nursing field, one who is a nurse manager shows great leadership. The reason why nurse manager plays an important role in patient care is because it is known to be the most difficult position. As a nurse manager, one must deal with many patient care issues, relationships with medical staff, staff concerns, supplies, as well as maintaining work-life balance. Also, a nurse manager represents leadership by being accountable for the many responsibilities he or she holds. Furthermore, this position is a collaborative yet vital role because they provide the connection between nursing staff and higher level superiors, as well as giving direction and organization to accomplish tasks and goals. In addition, nurse managers provide nurse-patient ratios and the amount of workload nursing staff has. It is their responsibility to make sure that nursing staff is productive and well balanced between their work and personal lives.
The nursing profession has changed drastically over time. The roles and responsibilities that nurses take on have increased and become far more complicated. Nurses are managers, leaders, supervisors and have become experts in many areas of care. Every day nurses are faced with the task of improving and strengthening professional leadership within their work environment. Managing good quality and eliminating risk is the major challenge in health care. All members of the team must work together to accomplish outstanding patient care. Budget cuts and nursing shortage in all areas of health care leads to less licensed staff, where use of unlicensed personnel have been used widely, where delegation is not an option, but a necessity. Nurses must be aware of delegation guidelines, what tasks to delegate,when to delegate for the safety of patients, liability of nurses and the facility.
This journal is a reflection of my experiences and lessons learned thus far within this course. Upon my reflecting, there were lessons that stood out the most. Also, there are nursing practices in my associate’s degree of nursing program (ADN) that correlates with the standard of care that was recently discussed. This reflection also prompt me to consider more in depth the necessary skills and attributes to become a competent nurse.
