As a pharmacist, I took an oath to; “…devote my professional life to the service of all humankind, consider the welfare of humanity and relief of human suffering my primary concerns, apply my knowledge, experience, and skills to the best of my ability, maintain professional competency, maintain the highest principles of moral, ethical, and legal conduct, embrace and advocate change in the profession of pharmacy that improves patient care, and fully realize the responsibility with which I am entrusted by the public...” (“Oath of a Pharmacist,” 2015). Based on the aforementioned oath, I personally believe the improvement in medication use and the advancement of patient care, such as the collaboration of healthcare professionals, can benefit the outcomes of population health. Population health not only includes the health outcomes of a group of individuals, but the factors and determinants that have an impact on the individual and population health outcomes.
My responsibility as a pharmacist is to advocate for my profession. In my current position, I have the opportunity to demonstrate to other healthcare professionals such as physicians, nurse practitioners, and physician assistants the ability to provide direct patient care and recognize the most optimal therapy for individual patients. According to a recent comprehensive systematic review of 298 research studies, “…integrating pharmacists into direct patient care results in favorable outcomes across health care settings and disease states” (Chisholm, 2010). In the realm of pharmacy, the processing of paper prescriptions has costed more than just time, but patient lives. “It’s estimated that medication errors contribute to more than 7,000 deaths annually” (“Electronic Prescription,” 2015). E-prescribing reduced communication and medication errors, ensures better patient medication compliance, provides clearer prescription and audit trails, authenticates prescribing sources, and reduces the time spent for clarification and refill callbacks. Overall, both of these programs will reduce inefficiency and increase productivity.
In conclusion, population health will never be perfect, but with the continued drive for optimal healthcare through technology, education, communication, and patient safety, there will exist better patient outcomes and satisfaction within our communities.
