As an intern with Volunteer Services, I have acquired experience administering and reviewing background checks for adults 18 and over. I also managed the volunteer inbox and database. In addition, I responded via email to any questions or concerns that applicants or parents had in regards to volunteering. With the organization Donor Relations, I managed donations from donor. I entered information into the company’s database and created bookplate templates as specified by donors. Updating spreadsheets with the number and percentage of attendees at the seminars was also an important part of my job
Area I have selected for the field is Long-Term Acute Care Hospital (LTACH) specializing in pediatrics. Long-Term Acute Care Hospital, specializes in treatment for special needs such as patients with concurrent acute illnesses, recovering from acute illness due to multisystem failures, requires direct care by multiple specialists, ventilator services (weaning or decannulation), needing of daily physician visits or have complex respiratory-relevant diagnoses. Which in terms focuses on patients who have complex medical problems and the rehabilitation needs that one may require special or intensive treatment over an extended period of time. Patients who come to LTACH have been in an ICU or CCU at a hospital and now require specialized care (as
Client is fully aware that completion of the assigned therapy program is strongly encouraged to be released from skilled nursing with a low possibility for retention or return. He presented with mild cognitive impairment and respiratory failure. Client demonstrated an adequate understanding of his condition and treatment recommendations. Client identified his older sister, niece and daughter as primary caregivers and supporters in case of financial or medical emergency. Client noted older sister as most significantly attentive to his needs. A care plan meeting was scheduled to meet with members of his immediate support system. Client medication upkeep will be closely and frequently monitored as he is on serious life sustaining medicines which need to be administered in a timely manner. Frequent rounds to client room, and follow-up assessments will be conducted and documented to track progress of respiratory rehabilitation. Rehabilitation will include nutritional, recreational, basic breathing and exercise training, specialized care targeting acute care needs. Client is new to the older adult living community therefore care planning will also include and attend to acclimation to daily living at a skilled nursing
A team led by RN, should mentally prepare their patients to understand their responsibility towards good health. This can be done by showing them special documentaries during their stay in the hospital, in a common room where other patients can also join them in a group of six to twelve. After the session, patients should be given a short comments form with multiple choice answers (Appendix A). The purpose is to check their positivity towards the message conveyed through the documentary. At this time patient's vitals should be checked and recorded for the future
(AC 1.2, 2.2) The care plan will be supported by team members including professional and non-professional members of staff. Therefore communication is essential to maintain the highest possible level of Jane’s well-being. Members of the team include nurses, GP, doctors, staff from the oncology unit who assist during chemotherapy and non-professionals such as personnel staff (cleaners, staff form reception), care workers, family members. Team members will provide help in different aspects on each stage of her illness. Some of them are to support her family (children: care workers), others to help in everyday life in hospital (care assistant, cleaners). However the nurse is the person who integrate this team and the coordinator during all cancer treatment. Working under supervision provide a nurse with learning opportunities also according to Royal College of Nursing (2002): ‘’Clinical supervision aims to motivate nurses, while being client-centred and focussed on safeguarding standards of client care’’.
It’s very important that the units that are supportive of evidence based practices instill the idea of the patients being as important as family members or close friends in order to carry out the best practice. These would be good areas for individuals of the team to reflect during meetings. What is means to care for patients as a close friends or family members and what areas they can improve their practice; For instance, long term care nurses taking the time to ensure that all of their bedridden residents are turned every 2 hours to prevent bed sores.
To secure as position as an Adult/Gerontology Acute/Critical Care NP in which my advanced education, training, and skills may be effectively utilized towards enhancing patient outcomes through interdisciplinary collaboration and continuing education to remain knowledgeable of research findings and practice recommendations in order to establish and execute the most appropriate evidence for practice as well as evaluate outcomes to create guidelines and improve clinical practice.
I joined the unit council and I’ve participated in the clinical ladder program at my facility. I believe all of these accomplishments will aid in progressing to the next level. I strive to have several experiences under my belt and I would like to be viewed as a hot commodity within the nursing field so in addition to hospital setting nursing I work part-time at a prison and a nursing home. The extra money helps pay for school and I plan to continue my education ever further. In addition to basic life support I am also certified in advance cardiovascular life support. I believe that I have some qualities of a proficient level nurse already. When caring for my patients I always go with my instinct. There have been numerous times that I’ve assessed patients and I couldn’t put my finger on what was wrong but I utilized my resources for a second opinion. I would have my charge nurse or department based educator step in to assess and give me their opinion as well. These patients were slowly deteriorating and I was able to get them the adequate help they
Likewise, in CNO Practice Standards, the nurse demonstrates knowledge application by identifying and recognizing abnormal or unexpected client responses and taking action appropriately by using or not using restraint. Using pre-existing knowledge in conjunction with observational and critical thinking skills, the nurse must be able to assess a patient’s condition and determine the most appropriate course of action. Every patient’s situation is distinct and unique; the nurse needs to be able to respond to that difference and make an appropriate
Matching staff skill to patient and family needs highlights the difference between delegation and assignment” (Weydt, 2010, Delegation and Related Concepts, para. 1). This principle came handy, when I was assigned to care for a patient who had multiple injuries from a car accident. The care was complicated with the presence of a Foley catheter, total parenteral nutrition (TPN), chest tube, wound vacuum, and ventilator machine to support the patient’s airway. Under normal patient care assignment, the nursing assistant would perform the bedside care, e.g., bed bath, turning and repositioning of patients who were on bed rest, assisting with feeding, to mention a few. However, for this particular patient, the medical conditions were complicated by the multiple machines and equipment that nursing assistants were not trained to manage. Hence, I was fully engaged in most of the patient care activities. My presence and involvement were required from the provision of bed bath to turning the patient every two hours. I made sure that even with the simplest activity, the safety of the patient was not compromised. The only task that I allowed the nursing assistant to perform on her own, was the emptying of the urine drainage bag for the output

I hope to impact this professional with my knowledge and expertise. I am now in school again to further my knowledge of this profession. It may seem old for some. However, for me learning has no age and boundaries as I continued to gain more knowledge and perhaps someday I can be a mentor for my peers. I hope to someday work In Risk management where I can continue to fight and argue the rights of my patients and peers whom all deserve a sense of dignity and self-worth. My contribution to this profession includes providing and promoting care with a holistic approach that is inclusive of the mind, body, spiritual belief practices because the patients I see as being more than just another patient in need of my care. The patients are like you and I and perhaps wants the same as I want for them. I’m sure with my great sense of enthusiasm that I may accomplish something here because my day will never end until I make an unmistakable stamp on a profession I always wanted to practice since that little girl whose heart was touched by the many caring professionals that provided care to my mom. Sound like I’m on a mission that indeed I

Due to working with people around in the university Colorado hospital, I believe I am equipped, ready, and determined to dedicate my life to this high level of responsibility. The leadership opportunities allowed me to develop and practice certain characteristics required for nursing. In the Cardiothoracic Intensive Care Unit it has trained me to overcome emotional discouragement, be patient, be attentive to others’ needs before my own, be available for others, build and maintain relationships, organize events, and manage my time under a busy schedule. Moreover, knowing the competitiveness and rigor of nursing school, it has motivated me to maintain disciplined study habits to precede my goal to become a nurse. While working in the Cardiothoracic Intensive Care Unit for two years, my experience at this hospital became the most rewarding and enjoyable. The level of complexity and the demand of taking care of the patients in the hospital contributed to my strong passion and urgency to continue to grow my knowledge and help patients. I am fortunate enough to say that all my hardships, triumphs, and various experiences have contributed to my desire and determination to meet the demands of the nursing
