1. Orientation x4- Patient was alert and orientated to person, place, time, and situation. When asked the patient was able to state his name, that the current year is 2016, that he was in South Beach Psychiatric Center on Staten Island and that he is admitted for mental illness (schizophrenia and bipolar).
2. Sensorium- Patient was alert, a little drowsy due to him yawning, and was lethargic due to putting his head down and stating “I am tired,” and was not hyperactive.
3. Patient’s appearance is unkempt. His clothes are dirty, had holes in the bottom on his pants and his shirt had stains. The patient’s memory was intact including his recent and remote events. He was able to recall several events in his life; his childhood, how he was sent to the foster home, the death of his mother, when he started to smoke weed, dropping out of high school, the recent fight with his brother which led to him getting kicked out of the brother’s house, and his arrival to SBP. The patient’s memory was tested. I asked him to remember 3 things: pencil, bird, and house. His immediate recall was 3/3 and short-term recall was 3/3.
8. The patient had hallucinations. The patient was had auditory and visual hallucinations. Patient stated that God was telling him to become on his priest. Patient stated that he sees the devil and his angles.
9. The patient’s insight was good. The patient was aware of their illness; he knew what mental illnesses he had and when he was diagnosed with him. The patient was aware of his responsibility and how his behavior affected others. He was aware of him and his brother getting into a fight for him smoking weed and leading to his brother kicking him out. He stated that it was his fault and he should not have smoked weed and fought his brother.
