As a healthcare professional, there are many different principles that should coincide with patient care. However, two of the main components in the delivery of competent patient care are quality and safety. Patients and families who spend time in the hospital setting are often in a very vulnerable state, however, they almost always remember the care that is delivered during this time. There are several different factors that may affect the delivery of safe care. One of the main issues regarding safe care is medication errors. In the United States, medication errors account for nearly 7000 deaths and affect up to 14% of patients (Haw, Stubbs, & Dickens, 2014, p. 798). Therefore, the main focus of this paper will be to develop an understanding of medication errors, common causes of medication errors, ways to prevent these occurrences, and how different professionals view medication errors. …show more content…

Therefore, physicians may not feel they are at the wrong when it comes to the issue of medication errors. An important topic that has an effect on patient safety is communication in the health care field. A study by Malone (2016) evaluated interactions between physicians with other health care disciplines. The study found that 88% of nurses, pharmacists, and other disciplines experienced poor communication interactions with physicians within their daily interactions (Malone, 2016, p. 157). Another 87% of individuals who provided answers to the study felt that physician was rushed during the call (Malone, 2016, p.157). If a nurse is fearful of speaking to a physician, this can lead to lack of calling for verification on a medication as well as lack of asking a physician to repeat an order over the phone.
