Level 4A Reflection Paper Level 4A has allowed me the chance to engage in various clinical experiences that have helped master my nursing skills. This semester I was assigned to Select Specialty. This clinical site is specialized in caring for complex medical conditions, such as patients who were on mechanical ventilation equipment’s, complex wounds, cardiac diseases, prolonged surgical recovery, long-term IV antibiotics, patient requiring dialysis support, renal failure, pre and/or post-transplant, post injury, neurologic diseases, uncontrolled diabetes and infections disease.

The patients at Select are very complex while I care for them, I use the nursing process to integrate assessment skills. Select gave me the opportunity to check blood sugars and documented, head to toe physical assessment and documentation and sepsis screen. Also, I took care of a patient with a colostomy and did Ostomy care, complete bed bath, oral and PEG tube medication administration, documented medication administration in the HMS computer system. Including, subcutaneous medication (insulin), insertion of a female catheter, wound care and documentation, intake and output documentation, sterile suctioning and in-line suctioning, care of a vent patient and observed post mortem care. Off unit experiences, going to the operating room and seen a surgical operation. Wound care department, I learned more about various type of procedures for healing injuries, including the Hyperbaric chamber. ICU department, shadowing and assisting a nurse caring for patients in critical conditions and Radiology I saw thoracentesis procedure. I had opportunities to ascertain more about South Milwaukee community related to health promotion, maintenance, and disease management. Our group had to do windshield survey, interviews, look for common health issues, at the same time use our knowledge of the nursing process to perform an assessment of the community. We found out after the interview and analyzing data, that the populations of South Milwaukee issues are high blood pressure and mental health disorders. Our nursing intervention was to implement the promotion of physical health, South Milwaukee has recreation centers and a senior center next to the city hall that extends a variety of classes and activities especially for the middle-aged population that is mostly seen in this city. Get the community more involved in physical exertion and physical classes and activities on a schedule based on their availability. Also, nurses should listen to the community stories about their illness
