Leptospirosis
Leptospirosis is disease that is seen worldwide in both humans and animals. Leptospirosis can affect most mammals and has a multitude of clinical signs or symptoms and has varying degrees of infectivity. It can be tricky to diagnose, can be fatal, and has the potential of infecting humans. Leptospirosis is a disease that is becoming increasingly prominent in the world and has even been causing epidemics in countries because it is frequently misunderstood. For these reasons alone it is vital to know how to help diagnose, treat, and prevent this disease in our veterinary patients.

People and animals are infected after being in contact with the urine of an infected person or animal. This includes being in contact contaminated water, bedding, soil, and food.1 The pathogenesis of Leptospirosis is quite simple, the Leptospires that are shed by an infected animal or person raid the body after entering through mucous membranes or open skin.3 The incubation period can vary from a few days to a couple weeks depending on the amount of bacteria that has invaded the body. In severe infections, the incubation period is shortened and in mild infections it can take the bacteria significantly more time to cause clinical signs. The severity of the disease also varies by the serovar and by the infected patient.3 After the incubation period, the leptospires circulate in the peripheral blood and most commonly replicate in the kidneys, liver, and lungs. This replication is what can cause severe complications in the body systems of animals that are infected with Leptospirosis. The amount of damage causes is dependent on the immunity of the infected animal and the how virulent the bacteria
Owning a dog comes with risk and with those risk are diseases, Leptospriosis is one of those diseases. Leptospirosis “has been attributed to more than 200 pathogenic serovars from the genus Leptospira” (Raghavan).” This disease brings deaths to dogs across the world every year and can be prevented by proper care and proper knowledge of the disease. The disease is not always prevalent in symptoms but the canine is the primary carrier of this disease, creating a large problem with minimal to do about controlling it. With a seasonal prevalence of canine leptospirosis cases, the disease can be evaluated and applied to future procedural warnings for owners.
In addition to its traditional clinical manifestations, GAS can also cause serious invasive disease such as necrotizing fasciitis, colloquially known as the flesh-eating disease. First broadly reported during the Civil War, when it was known as gangrene, necrotizing fasciitis occurs when an individual’s subcutaneous fat and superficial fascia become rapidly necrotic. Though incidence data is limited, one study estimated that, worldwide, there are approximately 660,000 cases of invasive GAS disease per year, with 97% of those cases occurring in low-income populations (4). Many microorganisms other than GAS have been linked with necrotizing fasciitis, including Staphyloccocus aureus, Escherichica coli, and Klebsiella pneumoniae, and the disease is often caused by a polymicrobial infection. However, the most well known causative agent in necrotizing fasciitis cases is usually Group A streptococci (6). Acute epiglottis is an infectious disease of the epiglottis and supraglottic structures that can cause sudden fatal airway obstruction. Airway management can be unpredictably difficult and challenging particularly in the event of the unexpected difficult airway. In rare cases, it is impossible either to intubate the trachea or to ventilate the lungs via mask. By anticipating these challenges and choosing the correct rescue strategy in managing the high-risk airway can increase the likelihood of a successful outcome. We report a case of a 41-year-old man presented with severe upper airway obstruction following acute epiglottis that urgently needed artificial airways. The complexities and the challenges encountered in managing the patient’s airway are discussed.
Symptoms of elephantiasis are enlargement and swelling of a part of the body due to the blockage of the lymphatic nodes. The lymphatic system is not able to take out the extra fluid of the body which causes an accumulation of body fluid. The arms and legs are the most affected areas in the body. It can swell to more than three times of its normal shape. Affected areas will have malformed shapes; skin and tissue will become thick and appear to look like an elephant’s leg. The skin of the affected areas becomes extremely dry, thickened (hyperkeratosis), and discolored. Other symptoms may include Fever, chills, and a feeling of sickness. Elephantiasis may affect the male and female external genital area. Some male suffer from enlargement of the scrotum, in some cases scrotum can become abnormally enlarged and can weigh over 100 lb. The penis may be hidden under the skin. In some women the external area of the genitalia will appear thickened and ulcerated rough skin may develop. The breasts may become enlarged. Infected individuals are susceptible to bacterial and fungal due to the damage of the lymphatic system. Infections may become worsen due to the lack of immunity caused by a damaged lymphatic system. Necrotizing fasciitis, known commonly known as "flesh eating bacteria [infection]", occurs in a wide range of people 1. It occurs in the elderly, middle aged and younger patients. It occurs in athletes and debilitated individuals. It can occur in drug abusers with self inflicted wounds and healthy individuals with incidental injuries. It may occur in those with metabolic disorders such as diabetes and those with suppressed immune systems. It may occur in those with no underlying disorders and no known particular injury. It occurs under ordinary circumstances such as a seemingly harmless cut or scrape and in wounds resulting from major trauma such as an auto accident.
Tuberculosis is an air-borne disease, hence, it can be passed from an infected person to a healthy individual through coughing, sneezing and other salivary secretions. The majority of clinical cases of laminitis occur at pasture where there is an accumulation of rapidly fermentable non-structural carbohydrates (NSC) such as fructans, simple sugars or starches (Geor, 2010). Pasture-associated laminitis has major economic and welfare implications in the equine sector. Increased risk factors include insulin resistance, increased insulin secretory response, hypertriglyceridaemia and obesity (Asplin, et al., 2007;Carter, et al., 2009 and de Laat, et al., 2010). Insulin resistance has been associated with a number of problems in the horse, most notably laminitis. Insulin resistance can be defined as a physiological condition in which cells have a diminished response to normal actions of the hormone insulin. Insulin is produced but the cells become resistant and are less capable in transporting glucose from the bloodstream to muscle and other tissues. In horses, insulin resistance is associated with a number of diseases such as Equine Metabolic Syndrome (Powell, et al., 2002; Hoffman, et al., 2003;Vick, et al., 2006 and Frank, et al., 2009), Equine Cushing Disease (McGowan, et al., 2004 and Walsh, et al., 2009) and Laminitis (Treiber, et al., 2006;Bailey, et al., 2007;McGowan, 2008 and Geor, 2008). What started as a problem with a horrific disease, lead to isolation of leprosy patients. It was hard for these patients to settle and make homes; communities feared the spreading of illness. The government took an old plantation to create a hospital for the leprosy patients. The old plantation was called hospital #66 or better known as Carville. “Over a long time period, the disease can be disfiguring, and societies have stigmatized victims of the disease. This attribute is deeply discrediting since the stigmatized individual is disqualified from full social acceptance. Tuberculosis or TB is an airborn infection caused by inhaled droplets that contain mycobacterium tuberculosis. When infected, the body will initiate a cell-mediated hypersensitivity response which leads to formation of lesion or cavity and positive reaction to tuberculin skin test (Kaufman, 2011). People who have been infected with mycobacteria will have a positive skin test, but only ones who have active TB will show signs and symtoms. Basic signs and symptoms include low grade fever, cough with hemoptysis, and tachypnea. They may also show pleuristic chest pain, dyspnea, progressive weight loss, fatigue and malaise (Porth, 2011).
