Review of Research: Everyday Excellence: A Framework for Professional Nursing Practice in Long-Term Care

A nursing home is normally the highest level of care for older adults outside of a hospital. Nursing homes provide what is called custodial care, including getting in and out of bed, and providing assistance with feeding, bathing, dressing and medication needs. However, nursing homes differ from other senior housing facilities in that they also provide a high level of medical care. Skilled nursing care is available on site, usually 24 hours a day. Other medical professionals, such as occupational or physical therapists, are also available. The economic impact on healthcare has taken its toll on the number of registered nurses providing bedside care to patients, compromising patient safety and increasing the potential for negative outcomes. Several factors have vastly contributed to the nursing shortages over the years, including healthcare organizations downsizing, increased workloads, inadequate staffing plans and job dissatisfaction. The label “nursing home” has negative connotations for many people. Yet nursing homes provide an important component of senior housing options.

Facilities that follow a professional practice model are setting the standard for optimum level nursing care. Residents are entitled to safe, quality care. Nurses are there to provide that care and to act as the patient advocate. Success in improving the quality of care in nursing homes will come from a combination of dimensions as portrayed in a Professional Practice Model. These models offer a promising approach that address the need to improve in the clinical practices throughout the facility as well as strengthening the frontline of nursing care in long term care facilities. Every facility needs to have a backbone to how they operate and by using a Professional Practice Model it creates just that; it creates a baseline for superior patient care, every day, through leadership and staff collaboration.
