Additional factors that are influencing the patient centeredness and provider patient communication include:- the Language barriers, which promote the racial and ethnic concordance and differences between the patient and provider which will affect the cultural competency and effect the disabilities on the patients’ health care experiences. Some of the health care services can expose the patient to some of the risks of unintentional injuries. Hence there are efforts to minimize these injuries that have led to the patient safety movement. To ensure that the health care is safe, efficacious and of high quality levels, it is very vital that there will be an optimal coordination of both the structural and cultural elements of the health care system. Unfortunately these requirements are not always met and fulfilled, thus this can lead to medical errors, whilst engaging to the concept that patient safety is threatened. The sources of errors can be quite diverse, including the faults in the medical equipment, and lack of team work within the health care professionals. Therefor patient safety is dependent upon the optimal interactions upon the components of the health care system, whilst ensuring that the errors are minimized. Medical errors are defined as the failure of a planned action to be completed, or the act of using the wrong plan to achieve the aim as intended.
