Introduction
In the essence of formulating community health assessment tool it helps in managing and preventing illness within the population of a specific area. It helps to address in a constructive manner to gather data in return will help health care professionals to provide and to have awareness of the health care needs that is relevant to the community health. Another central goal is to help the health care professionals to recognize maximum benefit of the assessment for the community. During the windshield survey assessment tool within in Nassau County as an aggregate the residents of county were found to wealthy and fairly healthy, nonetheless health imbalance or disparities exist in the community. Health imbalance are reflected in distinction of extend of life, quality of existence, the degree of disease, disability, morbidity and mortality, and the access of treatment will be the guideline to concentrate on the demographic and epidemiological data that are present in Nassau County, New York.
Community
Nassau County is located in the western region of Long Island, on the west side it is bordered by New York City’s Queens County and on the east side Suffolk County. The population is composed of 1,339,532 residents. Nassau County has a total of 453 square miles in which 287 square miles where the population resides, the rest of which is composed of water. Nassau County contains three towns and two cities from which Westbury, Hempstead, Freeport, Glen Cove are located within just to name ...
