Gout is a type of inflammatory Arthritis and is the most common form, it typically results from genetic and environmental factors. Evidence suggests that cases of Gout are getting increasingly more common, and that numbers have continued to grow in recent decades. Currently Gout affects approximately every one and two in every 100. Approximately 1 in 40 people in the UK are affected. While Gout is more prevalent in men, women can also be affected. In historical times it was linked to rich lifestyles heavy in meat and alcohol, it was often referred to as “disease of the kings” for this reason. In recent times it has been discovered which particular foods and lifestyles trigger gout. In most cases it is caused by too much uric acid in the blood,. An excess of uric acid is also known as hyperuricemia. This surplus of uric acid can lead to a build up of fluid surrounding the joints which form small crystals inside and around the joints. These crystals cause intense pain, inflammation and swelling, this can be very debilitating to the sufferer. Symptoms of gout develop very quickly over a period of hours and can remain for between three and ten days, after this attack the joint usually returns to normal. It is caused when the cartilage in the joints breaks down, causing the bones of the joint to rub against one another. This causes pain, stiffness, and loss of motion in the joint. Although the cause is unknown, it is believed that the increasing age of the population, prevalence of obesity and injuries add to the progression of the condition. This condition may be asymptomatic, or may present symptoms including soreness, stiffness and pain. The symptoms are more common in the older population, those with limited activity levels, and those who are obese. The pathophysiological difference between rickets and gout is obvious. Rickets is a disease consisting of soft bone usually in children. The medical term for softening of the bone is osteomalacia and is the term used when describing rickets to a patient. The anatomy and physiology of rickets starts when the osteoid (bone appearance, structure) does not calcify correctly within the body. Inside the blood stream, vitamin D travels throughout the body until the liver is reached. When in the liver, vitamin D is converted to calcidiol (hardening substance) and pushed to the kidneys where the process ends. The kidneys transform calcidiol into the final form of vitamin D called calcitriol. Calcitriol absorbs calcium, magnesium, and phosphate. Rickets occurs when calcitriol is not present in the kidneys.

Rheumatoid arthritis affects around 400,000 individuals within the UK, it is the most relentless chronic form of arthritis, and for adults it is the second most common form in patients (1). “RA is more prevalent among women than men, and usually develops in the fourth to fifth decades of life” (A). Rheumatoid arthritis is severe as it is a form of autoimmune disease, this is where the body’s immune system attacks and damages working tissue, due to the failure to recognise the difference between foreign cell and normal cells. For example it damages the synovial membrane surrounding the knee, and bone erosion begins. This damage to the working tissue triggers an inflammatory response causing fluid to build around the knee (2). However it can happen in any joint, this is why it’s such a large problem as you simply can’t inhibit the effects of the immune system, as it would leave you extremely vulnerable to more life threatening diseases. If effective treatment isn’t applied it could be disabling to patients, the inflammation causes pain, stiffness, muscle ache, rheumatoid nodules (lumps of tissue) and tiredness (3).
