General Description
Appearance. Claire is a Caucasian woman with light brown hair and brown eyes. Claire is estimated to be in her late 30s or early 40s. She is married and has two children. Claire is estimated to be a housewife who takes care of her children since she mentioned that she took care of the kids at home and her husband worked. Claire’s Education status is unknown. Claire is estimated to have a medium body frame and build. No physical abnormalities or disability was noted.

Nursing Priority No.1: Assess and Prevent Suicide Behaviors
This is chosen as the first/ main priority since suicide can be life–threatening. Claire has a potential risk for self-inflicted and life-threatening injuries since Claire had history of prior suicide attempts and she thought that it would better for her husband and kids to live without her. She wanted to die in peace and didn’t like how her husband saved her life. She was depressive and she stated that she felt hopeless as well.
Intervention
Since Claire did attempted suicide the week before, the nurse should screen her for current suicide intentions. Screening should be done with tools that are appropriate for the patient in terms of age and other personal characteristics (e.g. mental health and) physical health history. (Adam, 2013 p.384) Nurse should then minimize the risk for suicide by following the suicide precautions in the hospital by creating a safe environment for Claire during the time she is actively suicidal and impulsive, self-destructive acts are perceived as the only way out of a situation. (Varcarolis, 2015 p. 473) (E.g. make sure bathrooms have breakaway fixtures, remove dangerous objects such as ropes, belts, or cords, and ensure that the patient doesn’t hoard medications.) She had difficulty of being a mother and wasn’t able to function and meet the role expectation as a mother. She had inadequate resources available and was pathological fatigue. She mentioned how tired she felt all the time during the interview.
Intervention
When Claire is at hospital, the nurse should minimize her responsibilities and seek effective coping skills during the time she is severely depressed. It can decrease the feelings of pressure and anxiety and minimize feelings of guilt. (Varcarolis, 2015 p. 161)
It is also as important for the nurse to involve Claire and her family in continue care planning after her discharge to recognize negative thinking and thoughts she may have and educate her to reframe and refute these negative thoughts. (Moorhead& Delaney, 1997 p. 143) Some of the useful effective coping skills that were learned in the hospital could also be helpful for Claire to practice at home. Nurse can suggest a number of community-based support groups since Claire found the support group to be helpful in hospital. (Varcarolis, 2014 p. 256)
