Failure caused by accidents or unintentional means should not be blamed, but put into place policies for the failure to not happen again. Early detection of problems is the key to preventing failures.
Reading the example of the space ship explosion really brought up the point that in some cases, failure is simply not an option because lives are at risk. There are failures that do cause death. Also, there may be more than one cause, so it is vital to find not only the root cause, but other problems that are related to the failure.
Lastly, it is better to be realistic than idealistic in the planned prevention stages of failure. Everyone wants a project to succeed, but employees should be realistic and think more about what could go wrong than what could go right.
