Once the labwork is return, the first Lovenox injection is given by the Lead ED RN. The ED RN then calls the anti-coagulation clinic, leaves a voicemail with the patient information and faxes the orders received from the provider. According to the website Pharmacy Career Options, “pharmacists are also people who are professionally qualified to prepare and dispense medicinal drugs.” Only the pharmacist may dispense the medication (Pharmacy Career Options). To prepare and dispense, the pharmacists must count and pour accurately (Pharmacy Career Options). Special instructions for this process include special storage, packaging, accurate labeling, and counting (Pharmacy Career Options). Pharmacists deal with multiple insurance companies daily (Pharmacists Career, Jobs, and Training).
The technicians prepare IVs, patient medicine trays, and sometimes packages them in the pharmacy so the nurse is able to scan the medications. Other settings include, retail pharmacy where one would interact with the customers, and the main job is to fill the prescription bottles, and sometimes mix up creams in the compounding lab. A Pharmacy Technician career caught my interest after high school. My mother was a nurse at the time, and I was trying to figure out what to do with my life. A career in the medical field sounded great to me, and I love to help people.
The nurse will ask the patient (if they are alert and oriented, or else a family member if available at bedside) if they recognize the drugs and know their indications. If the patient is from a HCA hospital (which is common,) the drugs will already be in their electronic record and the admitting nurse will review last time administered for each drug before calling the doctor to review, edit or continue the current drugs for the patient. The doctor here has the opportunity to change route, dose, timing, or give additional drugs here. Pharmacy then is to check for interactions and appropriate med times (like cholesterol meds in evening). Some drugs (usually the very expensive or rare) are not stocked by pharmacy and so it is a drug that is continued under an order labeled “okay for patient to take own drug”, which means the patient’s family can bring in the unavailable drug and nursing brings it to be reviewed and barcoded by pharmacy, or else the drug is discontinued.

There was failure in the room as well with the medication administration. Having the unit setup so that a patient identifier has to be scanned in order for the computer to acknowledge the administration of the medication will help as a reminder for nurses to check patients, drugs and dosage before administration. Having these tools in place will only be validated by occasional unbiased audits of the system. Lastly, for the unit to prevent further medication errors we will have to provide educational resources and instruction to the staff on the implementation of new policies. This can be achieved with a multi-faceted approach that includes e-mails, discussion at staff meetings, online modules and tests, and/or mandatory class
