What Are Jaylan's Goals

Goal 1: Jaylan will stabilize his symptoms and behaviors.

• QP met face to face with Jaylan.
• QP provided the purpose of this session which was to gather information for OHIO Scales.
• QP instructed Jaylan to rate the degree to which he has experienced certain problems in the past 30 days.
• QP probed Jaylan to share how satisfied he is with the mental health service he has received thus far.
• QP inquired how much stress is in Jaylan’s life.
• QP determined how optimistic Jaylan is about his future.
• QP directed Jaylan to share how satisfied he is with his relationship with his mother.
• QP reflected on ways problems might get in the way of his ability to do every day activities.

Goal 2: Jaylan will prevent an out-of-home placement.

• QP determined if Jaylan has any pending charges.
• QP allowed Jaylan to share how it feels to get arrested.
• QP gathered information on Jaylan’s criminal background.
• QP determined how Jaylan feels about getting off on his legal charges.

Goal 1: Jaylan will stabilize his symptoms and behaviors.

• Jaylan was in an excellent mood when the QP met with him for a session.
• Jaylan reported, “I have been yelling, swearing, using drugs, skipping classes, lying, and breaking rules.”
• Jaylan commented, “I am moderately satisfied with the mental health service and it is very different from the mental health service I received the first time.”
• Jaylan stated, “There is some stress in my life with my mom wanting me to stop smoking and me trying to graduate on time, but keep up with my peers.”
• Jaylan acknowledged, “My future looks okay if I can graduate.”
• Jaylan established, “My relationship with my mother is okay, but I don’t disrespect her or raise my voice at her because I do respect all that she does for me.”
• Jaylan chose getting along with adults outside the family, being motivated to finish projects, getting passing grades, ability to express his feelings, paying attention and completing

Goal 2: Jaylan will prevent an out-of-home placement.

• Jaylan shared, “I smoke daily or every other day depending on which friend has the blunt that day.”
• Jaylan reflected on both of his urinalysis coming back positive for marijuana use.
• Jaylan reported, “I purchase marijuana by selling my stuff when it is my turn to buy a blunt which is $10.”
• Jaylan denied being in a gang, but hangs out with gang members.
• Jaylan stated, “I have known the boys that I hang out with for a long time, so I don’t have to join the gang to be hang with them because they know me.”
• Jaylan established, “I don’t have any pending charges at this time.”
• Jaylan acknowledged, “The first time I got arrested I was nervous, but I’ve been arrested 3 other times, so I know what to expect now.”
• Jaylan commented, “I have been charged with breaking and entering and armed robbery.”
• Jaylan reflected on having a great lawyer; therefore, he is happy he was able to get off on the charges that with his lawyer
