Foreword
This report has been written to explore what the context of healthy communities is. The defined community will be introduced and a geographical map included. Gathered information of the specified community will be evaluated, data’s and statistics will be presented in tables. The focus will be on the health and social care needs of that community. Viewpoints of the residents of the chosen community will be summarized and discussed. Recommendation about observed issues will be implemented. Furthermore, theories and sociological aspects of diverse authors regarding notion communities will be included as they can support the findings.
1. Notion of community

The old Small Heath library, built in the Gothic-Jacobean style which opened in 1893 was also used as a public bath and contains today the largest mosque in West Midlands, the Green Lane Masjid (Library of Birmingham,2016). Small Heath is a part of Nechells ward, the South Yardley ward and Bordesley Green ward surrounded by the areas Bordesley Green, Saltley, Tyseley, Sparkbrook, Yardley, Hay Mills and Bordesley (LACT Limited, 2010). Personal health is extremely important to everyone around the world. But it is especially important to citizens of the United States of America. Being one of the leading countries in Health technology and also in food and beverage leaves most people choosing between living a healthy lifestyle and indulging in the varieties of food we offer. Across the country, many people are living with pre-existing conditions, living in food deserts, living below the poverty line and a long list of other factors that either hinders them from eating healthy or force them to eat healthily. When trying to live a healthy lifestyle in this country not only does the promotion of prevention matter, but also the promotion of Career and job opportunities matter just as much. In the United States, Money equals Power and money also equal the opportunity to create and live a healthier lifestyle.
A community is comprised of a group of goal oriented individuals with similar beliefs and expectations. Currently the term is used interchangeably with society, the town one lives in and even religion. A less shallow interpretation suggests that community embodies a lifestyle unique to its members. Similarities within the group establish bonds along with ideals, values, and strength in numbers unknown to an individual. Person centred care means basing the care and support of a person around them. Looking at things from their perspective, promoting their beliefs, preference, likes and dislikes. They are involved in the development of their support plans, risk assessments and what they want to achieve. They determine what they want and how they want things doing. It promotes their individual needs and what is important to them. We listen to the individual and find out about their wishes and look at ways of carrying this out as safely as possible. We work with the individual, their families and others to empower the individual and to promote independence in their lives and ensure that the individual is supported to maintain their lives as they
The theory used for this case study was effective, because it was able to get P to recognise that change need to occur so the support that she received would be effective. Reflecting on action the outcome of the assessment and intervention that was applied to P’s case I used supervision and to discuss concerns I had regarding this family with my PS. Supervision helped me establish my concerns and areas I required further support in application to theory in evidence based practice ‘blind self’.

Clinical social workers in the community strive to enhance and maintain psychosocial functioning of individuals, families, and small groups. They also focus on prevention of psychosocial dysfunction or impairment, including emotional and mental disorders. The perspective of person-in-situation (psychosocial context) is fundamental to clinical social work practice (Austin, Barr, & Coombs, 2006).
The one thing that I have always known I loved to do with my life is to help people. Cicero had once said that, “Non nobis solum nati sumus. (Not for ourselves alone are we born)”, a statement that I have always held close to my heart as it means that we are put on this earth for a purpose and one of those purposes is to be there for people who are asking for help, as we should not only worry about our own purpose in life but others as well. I am applying to the Faculty of Social Work for a Bachelor of Social Work at the University of Calgary because I want to learn the skills required to enable me to work with people, supporting them, and making a difference in their lives. My goal is to enhance the wellbeing of the vulnerable, oppressed, and impoverished individuals of our society. Essentially, my reason is driven on my aspiration to see others succeed in fulfilling their own potential, not only to gain happiness, but to see their lives as fulfilling and worthwhile for themselves. Although it feels somewhat cliché, my passion and desire for the field of social work originated from a genuine wish to make a positive difference in other people’s lives. I want to feel like I have done something meaningful, worth something, and that I sparked a light into a person’s life that needs it the most. By receiving the opportunity for a Bachelor of Social Work, I believe I can achieve this and become a supportive and compassionate member of society.
I want to pursue a career in social work because I want to help people who are suffering from mental illness and who are underserviced when it comes to getting help for mental illnesses. By working with a distressed person to become better, not only will I be able to enhance their life, but also the community will transform into a more positive place. I view transformation as a ripple effect, where changing the situation for one person eventually causes a change in the majority. This is especially true when it comes to mental illness and the stigma that surrounds it. I have been highly involved in mental health work on my college campus since my sophomore year. What is a social work? “Social work is a profession which promotes social change and problem solving in human relationships and the empowerment and liberation of people to enhance wellbeing. Utilizing theories of human behaviour and social systems, social work intervenes at the point where people interact with their environments. This is a community profile that aims to identify a specific health improvement issue within a local geographically determined community. ‘A community profile is an attempt to describe a particular community or neighbourhood. It uses a variety of different techniques to build up a picture of the community from a number of perspectives’ (Barnardos.org.uk, 2004) including several components of a community such as its demographic characteristics, patterns and trends i.e. its epidemiology in order to make comparisons between different localities to determine areas needful of specific health improvements with the goal of improving local people’s health and reducing health inequalities. A core definition of community as distinguished by Macqueen and Mclellan et al (2001) is ‘a group of people with diverse characteristics who are linked by social ties, share common perspectives, and engage in joint action in geographical locations or settings’.
The theme of community has always had a central and prominent place in social theory. A number of connected problems are at the heart of social theory. These related problems are often thought of as variations of the key problems of the relationship between ‘the community’ and ‘the individual’. (Browning et al., 2000) Communitarianism is a philosophy that emphasizes the connection between the individual and the community. While the “community “may be a family unit, it is usually understood in the wider sense of interactions between the community of people in a geographical location, or who have a shared history or interest. (Wiki, 2014) Thus, this philosophy, in this period, has been said to be prominent in a number of distinctive and time-specific ways. Community is just the whole range of that sort of group or institution-not the individual, not the family, not the state, not the market, but all the ones in between: churches, neighbourhoods, schools, clubs, kinship networks, associations etc. The concept of community refers to both a particular class of social entities, and to a particular range of social relations. (Browning et al., 2000) Some characteristics of this philosophy includes the fact that it is related to older theories of community such as Marxism, pragmatism, romanticism, ethical socialism, and strands of theology from the Jewish, Christian and other religious traditions. Secondly, a number of government-sponsored social policies have brought the term ‘community’ to a new prominence in political and social discourses- policies such as community care, community policing and community regeneration. Whether people realize it or not they live within some sort of community. The World Health Organization define a community as: “a group of people, often living in a defined geographical area, who may share a common culture, values and norms, and are arranged in a social structure according to relationships which the community has developed over time” (Shuster, 2014, p. 397).
