Introduction:
Trafalgar is a small rural town in West Gippsland Victoria, (one and a half hours from Melbourne) which has a population of approximately 4,635 people (Australian Bureau of Statistics [ABS], 2015). The main health issue that is prominent in the Baw Baw Shire and primarily Trafalgar is cardiovascular disease (CVD) (Trafalgar Local Stats, 2015). CVD creates enormous costs for the health care system as it places a heavy burden on individuals and the communities due to resulting disabilities. The Trafalgar community has a central part to play in improving health for the Trafalgar residents as community health enables practitioners, managers and policy-makers to identify those in greatest need of health services and then target services particularly to these concerns. If cardiovascular health is to improve for the Trafalgar population, health promotion programs need to work hand in hand with medical providers in these communities to improve cardiovascular health. Trafalgar has a high percentage of elderly people in the community which could indicate the high levels of CVD in Trafalgar as CVD is more common in the elderly with 18.1% of Trafalgar’s population being over 65 years old (Gippsland health online, 2015) & (Trafalgar Local Stats, 2015). Despite improving trends, CVD remains Australia’s biggest killer, accounting for 16% of the total disease burden and 11% of national health system expenditure (Heart Foundation, 2014).
